摘　　要：目的探讨Guided Care护理模式在老年慢性疾病管理中的应用效果。方法选择160例老年患者为研究对象,分为干预组和对照组,对照组实施常规护理及健康教育,干预组实施Guided Care护理方案,干预6个月后比较两组患者的日常生活能力、家庭功能、社会支持状况。结果干预组日常生活能力、家庭功能、社会支持得分高于对照组(P<0.01或P<0.05)。结论对老年慢性病患者实施Guided Care护理模式,提高了老年慢性病患者的日常生活能力,加强家庭功能和社会支持,改善老年慢性病患者的生活质量。Objective To explore the effect of Guided Care nursing model in the management of chronic diseases in the elderly.Methods Totally,160 elderly patients were divided into intervention group and control group,control group received conventional nursing and health education while intervention group received Guided Care program.After 6 months,the daily living ability,family function,social support scores were compared between two groups.Results Intervention group had higher scores than control group in the daily living ability,family function and social support （P 〈 0.01 or P 〈 0.05）.Conclusion The implement of Guided Care for elderly patients with chronic diseases can improve their daily life ability,strengthen family function and social support,and improve the quality of life.
