摘　　要：目的探讨H型高血压患者血清同型半胱氨酸(Hcy)水平与早期肾损害的关系。方法选择符合条件的高血压患者178例,根据Hcy水平将患者分为H型高血压组(Hcy≥10μmol/L)91例和单纯性高血压组(Hcy<10μmol/L)87例,并选取同期查体健康人90例作为健康组,比较3组性别、年龄、体质量指数、常规生化指标及血清Hcy、胱抑素C水平和尿β2-微球蛋白水平的差异,Pearson相关分析Hcy水平与各指标之间的关系,同时以Hcy为因变量进行多元线性回归分析。结果 H型高血压组患者年龄及血清胱抑素C、尿β2-微球蛋白水平均明显高于健康组及单纯高血压组(P均<0.05)。Pearson相关分析发现,H型高血压患者Hcy水平和血清胱抑素C(r=0.709,P<0.05)、尿β2-微球蛋白(r=0.646,P<0.05)、年龄(r=0.695,P<0.05)呈正相关,多元逐步回归分析显示Hcy水平与血清胱抑素C(β=4.725,P<0.05)、尿β2-微球蛋白(β=4.637,P<0.05)、年龄(β=0.213,P<0.05)存在线性关系。结论 H型高血压患者血清Hcy水平与早期肾损害具有相关性。Objective It is to investigate the relationship between Hey level and the early renal damage in patients with type H hypertension. Methods 178 hypertensive patients met the standard were divided into 91 cases of type H hypertension group （ Hey 〈 10 （μmol/L） , and 87 cases of simple hypertension group （Hey 〈 10 μmol/L） according to the different levels of Hey. 90 healthy people accepted body examination at the same time were selected as healthy group. The difference in gender, age,BMI,normal biochemical indexes, serum Hey, CysC and β2-MG were compared among the three groups, Pearson corre-lation analysis was used to analyze relationship between Hey level and these indexes, Hey as dependent variable to do multiple linear regression analysis at the same time. Results Compared with the healthy group and simple hypertension group, the age, levels of serum cysC , urine β2-MG were significantly increased in type H hypertension（_P 〈 0. 05 ） . Pearson correlation analy-sis showed that the levels of Hey and serum cysC （ r = 0.709,P 〈 0. 05 ）,urinary β2-MG （ r = 0.646,P 〈 0. 05 ）,age （ r = 0.695 ,P 〈0. 05 ） were positively correlated in the patients with type H hypertension ; multiple linear regression analysis showed that there was a linear relationship between Hey and serum cysC（ β=4.725 , F 〈0.05） and urine β2-MG（ β=4. 637 , P 〈 0. 05 ） ,age（ β = 0. 213,P 〈 0. 05 ） . Conclusion The level of serum Hey is related to the early stage of renal damage in patients with type H hypertension.
