复方甘草酸苷联合盐酸左西替利嗪对皮炎湿疹患者Th1/Th2型细胞因子水平的影响

摘　　要：目的探讨复方甘草酸苷联合盐酸左西替利嗪对皮炎湿疹患者Th1/Th2型细胞因子水平的影响及临床疗效。方法选取2015年2月—2016年6月于该院门诊确诊的湿疹、皮炎患者100例,随机分为联合治疗组（n=50）及治疗组（n=50）,联合治疗组给予口服复方甘草酸苷联合盐酸左西替利嗪,治疗组单独给予口服盐酸左西替利嗪,另随机选取同期于该院健康体检中心行健康体检者为健康对照组（n=50）。检测治疗前后IFN-γ、IL-2、IL-4和IgE水平变化。结果治疗组治疗总有效率为72.00%,联合治疗组治疗总有效率为82.00%,两组数据间比较差异有统计学意义（χ^2=3.21,P〈0.05）。两组患者Th1型细胞因子IFN-γ、IL-2水平低于健康对照组,Th2型细胞因子IL-4水平高于健康对照组,IFN-γ水平的降低伴有IL-2水平的降低,IL-4的升高伴有IgE水平的升高,皮炎湿疹患者存在Th1/Th2反应向Th2漂移现象。治疗后两组治疗组患者Th1型细胞水平较治疗前升高,联合治疗组升高较明显,差异有统计学意义;Th2型细胞水平较治疗前降低,联合治疗组下降较明显,但差异无统计学意义。结论复方甘草酸苷联合盐酸左西替利嗪治疗皮炎湿疹,优于单独使用盐酸左西替利嗪,可能通过IFN-γ、IL-2、IL-4和IgE水平变化起作用。Objective To assess the effects of compound glycyrrhizin united levocetirizine hydrochoride in the treatment of the patients with dermatitis and eczema on Th1/Th2 and related cytokines levels. Methods Select 100 cases of confirmed outpatients with eczema and dermatitis from February 2015 to June 2016,randomly divided into two groups with50 patients in each. combined treatment group with oral compound glycyrrhizin united levocetirizine hydrochoride,treatment group with oral levocetirizine hydrochoride. 50 cases in the same period in our hospital physical examination center of physical examination were randomly selected for healthy controls. To test the levels of IFN-γ、IL-2、IL-4 and IgE before and after treatment. Results The efficacy of the combined treatment group and the treatment group was 82.36% and 72.60%,with a significant differences（P〈0.05）. The levels of IFN-γ、IL-2 of two groups were lower than physical examination for healthy group, The levels of IL-4 was higher than physical examination for healthy group. The Th1 cytokines levels were increased in the two treatment group patients after the treatment,the combined treatment group increased obviously, the statistics had obvious difference; The Th2 cytokines levels were reduced in the two treatment group patients after the treatment, the combined treatment group decreased obviously, but no difference between the statistics. Conclusion Compound glycyrrhizin united levocetirizine hydrochoride was superior than levocetirizine hydrochoride in the treatment of the patients with dermatitis and eczema, may through the IFN-γ, IL- 2, IL- 4 and Ig E level changes.
