预防食管胃机械吻合术吻合口并发症的探讨

摘　　要：目的探讨预防食管胃机械吻合术吻合口并发症的应用体会。方法选取该院2007年3月—2016年4月由笔者主刀的食管癌手术患者94例,将其随机分为对照组(47例)和观察组(47例),两组患者在行食管癌手术中,分别采取不同的吻合方式,其中对照组患者采取手工吻合,观察组患者采取食管胃机械吻合,比较两组患者的胃食管吻合时间、总手术时间、住院时间和并发症发生率。结果经过治疗,观察组患者的并发症发生率为10.63%,对照组患者的并发症发生率为27.65%,观察组患者并发症发生率明显低于对照组患者(P<0.05)。观察组患者的胃食管吻合时间、总手术时间和住院时间分别为(14.29±4.37)min、(135.48±18.47)min和(20.46±2.37)d,其数据与对照组对比,均具有更为明显的优势(P<0.05)。结论食管胃机械吻合术在临床应用中,具有较好的效果,可明显降低患者的并发症发生率,缩短患者的胃食管吻合时间、总手术时间和住院时间,适合广泛推广。Objective To study the prevention of esophagus stomach mechanical anastomosis of anastomotic complication application experience. Methods Our hospital in March 2007 to April 2016 esophageal cancer surgery by the surgeon himself 94 patients were randomly divided into control group（47 cases） and observation group（47 cases）, the line of esophageal cancer patients were in, they were taken to different anastomosis, which take manual control patients anastomosis was observed in patients taking gastroesophageal mechanical anastomosis were compared in patients with gastro-esophageal anastomosis time, total operating time, length of stay and complication rates. Results After treatment,patients with complications in the observation group was 10.63% incidence of complications in the control group of patients with the incidence of 27.65%, the incidence of complications in patients in the observation group was significantly lower than control patients（P〈0.05）. Observation group were gastroesophageal anastomosis time, total operating time and hospital stay were（14.29 ± 4.37） min,（135.48 ± 18.47） min and（20.46 ± 2.37） d, comparing their data with the control group, have more obvious advantage（P〈0.05）. Conclusion Esophagogastric mechanical anastomosis in clinical use, with good results, can significantly reduce the incidence of complications in patients, shorten the patient＇s stomach esophageal time, total operating time and hospital stay, suitable for a wide promotion.
