Tei指数评价慢性肺源性心脏病患者右心功能的价值分析

摘　　要：目的探讨Tei指数评价慢性肺源性心脏病患者右心功能的临床价值。方法选取该院自2015年1—6月收治的40例慢性肺源性心脏病患者为研究对象,作为肺心病组,以40名健康人为对照组,所有研究对象均接受超声心动图检查,比较两组间右心房、右心室三尖瓣口舒张晚期和舒张早期(E、A峰)、A/E值以及右心室Tei指数差异。结果肺心病组患者RA[(5.42±0.71)cm vs(3.48±0.28)cm]、RV[(3.84±0.35)cm vs(1.79±0.21)cm]和MPAP[(31.46±4.56)mm Hg vs(20.32±3.15)mm Hg]均明显高于对照组患者,差异均有统计学意义(t=-3.245、-3.452、-3.156,P<0.05)。肺心病组A/E值[(1.42±0.42)vs(0.94±0.26)]以及Tei指数[(0.69±0.15)vs(0.35±0.08)]明显升高而EF[(0.49±0.05)vs(0.68±0.14)]明显降低,与对照组相比差异有统计学意义(t=-5.864、-7.546、4.672,P<0.05)。结论Tei指数是评价慢性肺源性心脏病患者右心功能的敏感指标,方法简单,在基层医院即可实现,值得在临床中推广应用。Objective To evaluate the clinical value of Tei index in evaluating right ventricular function in patients with chronic pulmonary heart disease. Methods 40 patients with chronic pulmonary heart disease patients admitted at our hospital from January 2015 to June 2015 were selected as the research object, 40 cases of healthy people were selected as control group, all subjects were undergone echocardiography. The right atrial and right ventricular tricuspid valve late diastolic and diastolic early(E, A), A / E value and right ventricular Tei index difference were compared between the two groups. Results The RA[(5.42±0.71)cm vs(3.48±0.28)cm],RV[(3.84±0.35)cm vs(1.79±0.21)cm] and MPAP[(31.46±4.56)mm Hg vs(20.32±3.15)mm Hg] of the pulmonary heart disease group were significantly decreased,the differences were statistically significant(t =-3.245,-3.452,-3.156, P < 0.05). The A / E value [(1.42 ±0.42) vs(0.94± 0.26)] and Tei index [(0.69 ± 0.15) vs(0.35± 0.08)] were significantly increased and EF[(0.49± 0.05) vs(0.68 ±0.14)] of the pulmonary heart disease group were significantly decreased, the differences were statistically significant(t=-5.864,-7.546, 4.672, P < 0.05). Conclusion Tei index is a sensitive index for evaluating right ventricular function in patients with chronic pulmonary heart disease. The method is simple and can be realized in the primary hospital, and it is worth popularizing in clinical practice.
