单腔气管插管麻醉联合人工气胸在微创食管癌切除术中的应用

摘　　要：目的:探讨单腔气管插管麻醉联合人工气胸在微创食管切除术中的应用。方法:对24例胸腔镜食管癌切除的患者临床资料进行回顾性分析,麻醉诱导时采用单腔气管插管,术中用CO_2建立人工气胸。结果:单肺通气时一过性低氧血症(SpO_2<90%)4例,通过调整呼吸参数,改吸入纯氧等措施,SpO_2逐渐回升至正常。手术均顺利完成,无麻醉意外及围手术期死亡。结论:单腔气管插管麻醉联合CO_2人工气胸微创食管癌切除程序安全可行。Objective To investigate the single-lumen endotracheal intubation anesthesia combined with artificial pneumothorax application in minimally invasive esophageal resection. Method 24 patients with clinical data thoracoscopic esophagectomy were retrospectively analyzed,using a single lumen endotracheal intubation anesthesia,using CO2 establishment of artificial pneumothorax during surgery. Results OLV transient hypoxemia（ pO2 〈90%） 4 cases by adjusting the respiratory parameters,to change the inhalation of pure oxygen and other measures,SpO2 rose gradually to normal. The operations were successfully completed without anesthesia accidents and perioperative mortality. Conclusion The single lumen endotracheal intubation anesthesia combined CO2 pneumothorax minimally invasive esophagectomy is feasible and safe procedure.
