彭江云辨治强直性脊柱炎肾亏督寒证诊疗特色

摘　　要：彭江云教授为吴氏扶阳学派第三代传人,擅长应用中医药诊治风湿性疾病,其认为强直性脊柱炎的关键病机为"阳虚邪凑",肝肾精血亏虚、肾亏督寒为本,风寒湿热等邪气乘虚深侵肾督之脉为标,内外合邪,痹阻气血,留滞于经络关节而成此病。对于强直性脊柱炎患者的诊治,提出温肾强督、蠲痹通络为基本治疗原则,善用并重用附子、细辛等温药以迅速缓解患者临床症状,重视扶正培本、扶阳存阴、温散寒湿、活血化瘀、顾护脾胃为特色;临诊强直性脊柱炎肾亏督寒证多使用附子桂枝汤加减,附子常用剂量为30~90 g,收效良好。Professor PENG Jiangyun is the third generation descendant of the WU＇s supporting yang school. Professor PENG excels at using traditional Chinese medicine in the treatment of rheumatic diseases and believes that the key pathogenesis of ankylosing spondylitis is ＂yang deficiency accompanied with pathogenic factor aggregation＂, kidney essence and blood deficiency, kidney deficiency and du meridian cold is the root. The pathogenic factor such as wind, cold, pathogenic, heat headed deep vein is invading the kidney supervises, inside and outside evil, stagnation of qi and blood, and the retention of the meridian joint. For the diagnosis and treatment of patients with ankylosing spondylitis, we regard supplementing kidney and strengthening du meridian removing arthralgia and dredging collateralsas as the basic treatment principle, use and reuse of monkshood and asarum isothermal medicine to quickly relieve the clinical symptoms, pay attention to therapeutic canberra centralizer,supporting yang and yin, warm and cold dampness, promoting blood circulation for removing blood stasis, temperature and dispersion characteristics for nursing care of the spleen and stomach; Clinical diagnosis of ankylosing spondylitis and kidney cold syndrome using ramuli cinnamomi and aconiti praeparatae decoction, common monkshood dose is 30~90g, often can receive a good effect.
