羚羊角胶囊联合丙戊酸钠治疗外伤后癫痫的临床研究

摘　　要：目的探讨羚羊角胶囊联合丙戊酸钠治疗外伤后癫痫的临床疗效。方法选取2015年2月—2016年10月在天津医科大学总医院滨海医院进行治疗的外伤后癫痫患者78例,随机分成对照组(39例)和治疗组(39例)。对照组口服丙戊酸钠片,15~20 mg/(kg·d),3次/d。治疗组在对照组基础上口服羚羊角胶囊,0.6 g/次,1次/d。两组经过4个月治疗后进行效果评价。评价治疗后两组患者临床疗效,同时比较治疗前后两组血清学指标、睡眠质量改善情况和癫痫发作频率。结果治疗后,对照组和治疗组的总有效率分别为79.49%和94.87%,两组比较差异具有统计学意义(P<0.05)。治疗后,两组患者血清白细胞介素-6(IL-6)、神经元特异性烯醇化酶(NSE)、脑源性神经营养因子(BDNF)和S100β水平均显著降低(P<0.05);且治疗组血清IL-6、NSE、BDNF和S100β水平明显好于对照组(P<0.05)。治疗后,两组患者AIS评分、ESS评分和PSQI评分均明显下降(P<0.05);且治疗组AIS评分、ESS评分和PSQI评分明显低于对照组(P<0.05)。治疗后,两组癫痫发作频率均比治疗前明显降低(P<0.05);且治疗组患者癫痫发作频率明显低于对照组(P<0.05)。结论羚羊角胶囊联合丙戊酸钠片治疗外伤后癫痫效果明确,可有效改善患者临床症状及促进神经功能恢复,具有一定的临床推广应用价值。Objective To investigate the clinical effect of Lingyangjiao Capsules combined with sodium valproate in treatment of post-traumatic epilepsy. Methods Patients（78 cases） with post-traumatic epilepsy in Coastal Hospital of General Hospital of Tianjin Medical University from February 2015 to October 2016 were randomly divided into control（39 cases） and treatment（39 cases） groups. Patients in the control group were po administered with Sodium Valproate Tablets, 15 — 20 mg/（kg·d）, three times daily. Patients in the treatment group were po administered with Lingyangjiao Capsules on the basis of the control group, 0.6 g/time, once daily. Patients in two groups were treated for 4 months. After treatment, clinical efficacy was evaluated, and the seizure frequency, serological indexes and sleep quality in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 79.49% and 94.87%, respectively, and there were differences between two groups（P〈0.05）. After treatment, the IL-6, NSE, BDNF and S100β levels in two groups were significantly decreased（P〈0.05）. And the serological indexes in the treatment group were significantly better than those in the control group（P〈0.05）. After treatment, the AIS, ESS and PSQI scores in two groups were significantly decreased（P〈0.05）. And these scores in the treatment group were significantly lower than those in the control group（P〈0.05）. After treatment, the seizure frequencies in two groups were significantly decreased（P〈0.05）. And the seizure frequency in the treatment group was significantly lower than that in the control group（P〈0.05）. Conclusion Lingyangjiao Capsules combined with sodium valproate has clear curative effect in treatment of post-traumatic epilepsy, and can effectively improve the clinical symptoms and promote the recovery of neurological function, which is worthy of clinical application.
