摘 要:目的探讨尘肺及合并肺部感染患者外周血T细胞的表达特性及T细胞的活化状态,分析其与疾病的关系。方法选择尘肺患者120例,尘肺合并肺部感染患者20例,肺结核合并感染患者42例和健康志愿者23例,采集患者肝素抗凝全血100μL,运用流式细胞术检测CD3+T、CD4+T、CD8+T细胞,CD8+CD28+活化T细胞及CD4+CD25hi调节性T细胞(Treg),同时采用统计学方法对结果进行分析和比较。结果与对照组、肺结核合并感染组比较,尘肺组及尘肺合并肺部感染组CD3+T细胞无明显变化(P>0.05);尘肺组、尘肺合并肺部感染组、肺结核合并感染组CD4+T细胞分别为(34.17±0.60)%、(33.11±2.36)%和(44.34±1.90)%,均明显低于对照组(49.94±1.72)%(P<0.05),且尘肺组、尘肺合并肺部感染组均明显低于肺结核合并感染组(P<0.05);尘肺、尘肺合并感染组CD8+T细胞分别为(27.85±0.66)%和(24.05±2.08)%明显低于对照组(42.15±1.18)%(P<0.05),而肺结核合并感染组则高于对照组(P<0.05)。CD8+CD28+的活化T细胞尘肺组、尘肺合并肺部感染组、肺结核合并感染组分别为(21.86±0.46)%、(20.35±1.82)%和(21.94±0.95)%均明显低于对照组(54.47±2.44)%(P<0.05),而CD4+CD25hi的Treg则明显高于对照组(P<0.05)。结论尘肺患者其疾病的发生和发展与T细胞亚群分布失衡密切相关,且其T细胞亚群的分布与结核并感染患者明显不同。Objective To evaluate T cells expressing characteristics and its activation status in peripheral blood of patients with pneumoconiosis and pulmonary infection,and to analyze its relationship with the disease.Methods 120 cases of pneumoconiosis patients,20 cases of pneumoconiosis combined with pulmonary infection,42 cases of tuberculosis patients combined with pulmonary infection and 23 cases of healthy volunteers were included.100μl heparin anticoagulated whole blood was collected from every patients,and the CD3+T、CD4+T、CD8+T cells,CD8+CD28+activated T cells and CD4+CD25hi regulatory T cells(Treg)were detected by flow cytometry.The results were analyzed and compared with statistical methods.Results Compared to the control,the tuberculosis and the infection groups,there was no significant change of CD3+T cells in pneumoconiosis patients(P〈0.05).The CD4+T cells in three experimental groups were significantly lower than the normal(P〈0.05),and also in pneumoconiosis patients significantly lower in pulmonary patients(P〈0.05).CD8+T cells in the pneumoconiosis and the infected groups were significantly lower than the control(P〈0.05),while CD8+T cells in the tuberculosis and the infection groups were higher than normal(P〈0.05).The activated CD8+CD28+T cells in three experimental groups were significantly lower than normal,while CD4+CD25hi regulatory T cells(Treg)were significantly higher than the control.Conclusions The occurrence and development of the disease in pneumoconiosis patients were closely related to the T cell subsets distribution imbalance,and the distribution of T cell subsets in pneumoconiosis was significantly different from TB.
