摘 要:目的分析脉波轮廓温度稀释连续心排出量监测(PICCO)技术在重症心脏病术后的临床应用价值。方法以2015年2月~2017年2月于我院接受心脏手术治疗的25例患者为研究对象,术后均行PICCO监测,分别在术后即刻与术后4h、8h、12h、16h、20h观察PICCO监测患者术后心排指数(CI)变化,分析GEDVI与CI间相关性。结果全心舒张末期容积(GEDVI)及心脏指数(CI)间无显著相关性(P>0.05)。GEDVI≤850ml/m^2时,GEDVI与CI间呈正相关(P<0.05);GEDVI>850ml/m^2时,GEDVI与CI间呈负相关(P<0.05);术后CI先逐渐下降,后又逐渐上升,以术后16h CI值最低。结论在重症心脏术后运用PICCO监测技术,可对心脏前负荷、心脏功能等指标进行监测,以明确心循环功能变化,可作为术后监测血流动力学一种新的有效方法。Objective To analyze the application value of pulse indicator continuous cardiac output(PICCO) monitoring technique in severe heart disease after surgery. Methods Five patients who underwent cardiac surgery in our hospital from February 2015 to February 2017 were selected as study subjects and were monitored by PICCO. Immediately,4h, 8h, 12h, 16h and 20 h after surgery, the changes of cardiac output index(CI) were monitored and the correlation between Global End-diastolic Volume Index(GEDVI) and CI was analyzed. Results There was no significant correlation between GEDVI and cardiac output index(P〈0.05). When GEDVI was ≤850ml/m^2, GEDVI was positively correlated with CI(P〈0.05).When GEDVI was 850ml/m^2, GEDVI was negatively correlated with CI(P〈0.05). After surgery, CI decreased and then increased, and it was the lowest at 16h after surgery. Conclusion The application of PICCO monitoring technique after cardiac surgery can monitor the cardiac preload and cardiac function indexes to observe the changes in cardiac circulatory function. It is an effective method for monitoring hemodynamics after surgery.
