摘  要:目的构建猪小肠黏膜下层脱细胞基质材料并用于修复大鼠腹壁缺损,与美国强生公司的ULTRAPROTM补片对比,观察猪小肠黏膜下层脱细胞基质材料(SIS)修复大鼠腹壁缺损后的大体效果及局部炎症反应。方法按照文献中报道的方法制作SIS材料并修复大鼠腹壁缺损(S组),并与美国强生公司的ULTRAPROTM补片(P组)进行对比,于术后3、8周取材,观察并评估2组大体修复效果及修复材料-肌肉交界处局部炎症反应。结果术后3周,SIS材料大体外观类似肌肉筋膜组织,缺损修复区域材料增厚;HE染色可见S组与P组大鼠腹壁缺损的修复区域均伴有炎症细胞浸润,SIS部分降解,部分胶原组织生成。术后8周,SIS材料边缘模糊不清,与周围正常肌肉筋膜组织相融合,缺损修复区域组织增厚明显;S组局部炎症细胞浸润程度明显减轻,SIS进一步降解,大量排列紧密、有序的成纤维细胞及胶原纤维增生。术后3周及8周,P组炎症细胞浸润的程度均高于S组,差异有统计学意义。结论 SIS材料修复大鼠腹壁缺损,效果显著,局部炎症反应轻微,并能够促进胶原组织增生。Objective To construct the small intestinal mucosa (SIS), and to evaluate the general effect and local inflammatory reaction in rat abdominal wall defect repaired with SIS, compared with ULTRAPROTM patch.Methods We constructed the SIS material by using the method reported in the literature, and repaired the abdominal wall defect with SIS (S group) via rat model, compared with the ULTRAPROTM (Johnson & Johnson, USA) as P group.At 3 weeks and 8 weeks after implantation, the specimen, including the biomaterials and surrounding muscle fascia tissue, was obtained to detect the inflammatory reaction with haematoxylin and eosin (HE) stain.And also, the general effect of repair was observed.Results At 3 weeks after operation, the material of SIS was similar in appearance to muscle fascia tissue, and the material in the defect area was thickened.Both S group and P group showed inflammatory cell infiltration within in the repair area of abdominal wall defect.SIS turned partial degradation with a small amount of collagen formation.At 8 weeks after operation, the edge of SIS material was blurred and fused with the surrounding muscle fascia tissue, and the defect area was obviously thickened.Meanwhile, the degree of infiltration of inflammatory cells in S group was significantly reduced, accompanied with further degradation of SIS and a large number of compacted and orderly fibroblasts and collagen fibers proliferation.Both at 3 weeks and 8 weeks after operation, the degree of inflammatory cell infiltration in P group was significantly higher than that in S group.Conclusion After repair of abdominal wall defect in rats with SIS, it is showed that SIS induced slightly local inflammatory reaction and could promote the proliferation of collagen.
