基层医院开展局麻下腹股沟Lichtenstein疝修补术临床分析(附31例报告)

摘　　要：目的探讨在基层医院开展与推广局麻下Lichtenstein无张力疝修补术的可行性及安全性。方法分析自2013年1月至2015年6月,新疆乌什县人民医院局麻下行Lichtenstein无张力疝修补术的31例腹股沟疝患者的临床资料,均在局部麻醉下使用同一种补片实施无张力疝修补术,观察患者围手术期恢复效果、术后并发症及对术中注意事项进行总结。结果本组31例患者均顺利完成手术,麻醉效果良好,未出现改变麻醉方式或加用静脉麻醉等情况,手术平均时间为55 min。术后胃肠功能恢复时间为4~12 h,住院天数3~7 d,平均4.35 d,术后无尿潴留,阴囊积液2例、切口皮下积液1例,术后慢性疼痛1例。术后随访12~30个月,目前无1例复发。结论在基层开展成人腹股沟疝局麻下Lichtenstein修补术是安全、简便、经济、有效的手术方式,以术后恢复快、手术指症广、麻醉相关并发症的发生率低等优势适合在基层医院推广。Objective To explore the safety and feasibility of Lichtenstein hernia repair technique under local anesthesia in primary hospital.Methods The clinical data from 31 cases of Lichtenstein tension-free hernia repair under under local anesthesia with same mesh in Xinjiang Wushi People′s Hospital from January 2013 to June 2015 were analyzed.The effect of perioperative recovery, complications and intraoperative considerations were investigated.Results All operations in this case series were successfully completed under local anesthesia, without changing the mode of anesthesia or combined intravenous anesthesia, etc, The average operation time was 55 minutes.The gastrointestinal function recovery time was 4 to 12 hours.The length of hospital stay 3 to 7 days, an average of 4.35 days.No urinary retention occurred, and there were 2 cases of scrotum effusion, 1 case of subcutaneous effusion, 1 case of chronic pain.There was no recurrence in the follow-up of 12 to 30 months.Conclusion Lichtenstein hernia repair technique under local anesthesia is a safe, simple, cheap and effective method for inguinal hernia.It has the advantages of fast postoperative recovery, wide surgical indications and low rate of anesthesia related complications, should be widely performed, especially in community hospitals.
