摘  要:目的:应用会阴超声观察女性前盆腔脏器脱垂器官运动及角量参数变化。方法:选取2016年6月至2017年3月在本院经临床诊断前盆腔脱垂患者100例(脱垂组)和正常妇女70例(对照组),会阴超声观察其在静息状态和最大Valsaval动作时前盆腔脏器的活动情况,测量不同时期膀胱颈到耻骨联合下缘水平线的距离(BSD)、膀胱颈与耻骨联合中轴线的夹角(α角)、膀胱尿道后角(β角)等角量参数。结果:脱垂组与对照组前盆腔器官运动方向一致,两组病人在不同时期膀胱颈下降度(BND)和α角比较,差异具有统计学意义(P<0.05),而组间β角比较,差异无统计学意义(P>0.05)。结论:会阴超声能动态观察前盆腔脱垂脏器的异常运动,间接反映盆底支持组织的功能状态,为前盆腔脱垂的诊断及功能评估提供了可靠信息。Objective To investigate the pelvic organs motion and evaluate the angular parameters variation in anterior compartment organ prolapse women with transperineal ultrasound. Methods A total of 100 patients(prolapse group) and 70 normal women(control group) were enrolled in this study from June 2000 to March 2017. Perineal ultrasonography observed in resting state and maximal Valsaval action before the pelvic organ activity. The distance(BSD) of the bladder neck to the lower edge of the pubic symphysis, bladder neck and pubic symphysis axis of the angle(α angle), bladder urethral posterior angle(β angle) and so on was measured. Results There was a significant difference between the two groups in the descending degree of bladder neck and the angle of α in different groups, and the difference was statistically significant(P〈 0.05). However, there was no significant difference between the two groups of β angle(P〉 0.05). Conclusions The perineal ultrasonography can dynamically observe the abnormal movement of the pelvic prolapse organ, and indirectly reflect the functional status of the pelvic support tissue, which provides reliable information for the diagnosis and function evaluation of the pelvic prolapse.
