新生儿高胆红素血症两种换血疗法的比较

摘　　要：目的比较经脐静脉插管单通道反复操作的Diamond氏法和经周围动、静脉同步换血的Wiener氏法的临床效果与合并症 ;方法将 2 2例高胆红素血症患儿随机分为两组 ,对照组 9例 ,观察组 13例。两种换血疗法从建立换血途径的难易 ,血清总胆红素下降率 ,血液动力学改变等合并症进行比较 ;结果Wiener氏法操作简便 ,无发生合并症 ;且血清总胆红素下降比对照组更明显 (P <0 .0 0 1) ;结论Wiener氏法优于Diamond氏法 ,值得推广。Objective To compare the differenty clinical effect and complication of Diamond method with those of Wiener method.Diamond method was that blood was transfused and drawed both through umbilicus vein again and again,while Wiener method was that blood was through vein and artery.Method Twenty two newborns with hyperbilirubinemia were randomly divided into two groups:control group and observe group.The effects of the two methods were based on the decrease ratio of the total bilirubin in serum, the change of blood dynamics, and difficult degree on building exchange blood way.Result Wiener method which is simple and non complication is superior to observe groups in the decrease ratio of total bilirubin( P <0.001).Conclusion Wiener wethod was superior to Diamand method It is worth popularizing wiener method.
