大竹县35～60岁农村妇女人乳头瘤病毒感染状况及其与宫颈病变的关系研究

摘　　要：目的探讨35~60岁农村妇女人乳头瘤病毒(HPV)感染状况及其与宫颈病变的关系。方法选择2013年6月-2016年6月大竹县5 000例35~60岁已婚妇女进行宫颈薄层液基细胞学、宫颈脱落细胞HPV-DNA以及阴道镜检查。结果5 000例受检者中,HPV感染总阳性率为29.66%(1 483/5 000)。受检者中无宫颈病变4 104例,宫颈鳞状化生245例,慢性宫颈炎442例,CINⅠ101例,CINⅡ64例,CINⅢ44例;宫颈癌前病变妇女HPV感染率分别为67.33%、78.13%和86.36%,明显高于其他受检者,差异有统计学意义(P<0.05);35~40岁、41~50岁和51~60岁组HPV感染率分别为29.84%、30.04%和29.33%,各年龄段比较,差异无统计学意义(P>0.05);首次性生活年龄≤20岁妇女HPV感染率为37.57%,明显高于其他年龄段组,差异有统计学意义(P<0.05)。结论大竹县35~60岁农村妇女HPV感染与宫颈病变和初次性生活年龄有关。Objective To explore the relationship between human papillomavirus（ HPV） infection status and cervical lesions in rural women aged 35-60 years old in Dazhu county. Methods Liquid-based cytology test,cervical exfoliated cell HPV-DNA tect and colposcopy were performed among 5 000 married women aged 35-60 years old in Dazhu county from June 2013 to June 2016. Results The total positive rate of HPV infection was 29. 66%（ 1 483/5 000）. Among 5 000 women,4 104 women were not found with cervical lesions,245 women were diagnosed as cervical squamous metaplasia,442 women were diagnosed as chronic cervicitis,101 women were diagnosed as cervical intraepithelial neoplasia（ CIN） Ⅰ,64 women were diagnosed as CINⅡ,and 44 women were diagnosed as CINⅢ. HPV infection rates in the women with cervical precancerous lesions were 67. 33%,78. 13%,and 86. 36%,respectively,which were statistically significantly higher than those in the other women（ P〈0. 05）. HPV infection rates in women aged 35-40 years old,41-50 years old,and 51-60 years old were 29. 84%,30. 04%,and 29. 33%,respectively,there was no statistically significant difference among different age groups（ P〈0. 05）. HPV infection rate in women with the age of sexual life for the first time≤20 years old was 37. 57%,which was statistically significantly higher than those in other age groups（ P〈0. 05）. Conclusion HPV infection in rural women aged 35-60 years old in Dazhu county is correlated with cervical lesions and the age of sexual life for the first time.
