舒适化医疗理念在胃癌患者围术期的应用实践

摘　　要：目的 探索舒适化医疗理念在胃癌手术患者围术期的应用效果.方法 选取2015年1—11月行胃癌根治术的患者238例,根据舒适化疗医疗项目开展的时间前后划分为观察组120例及对照组118例,对照组围术期行常规治疗及护理,观察组应用包括一系列以提高患者舒适化体验为核心的医疗理念进行护理.以术前焦虑评分、术后疼痛评分、术后并发症情况、术后肠道功能恢复时间、下床活动时间、术后住院时间、住院费用、并发症和满意度为主要观察指标比较两组的差别.结果 观察组在术前焦虑评分、术后疼痛评分、术后肠道功能恢复时间、下床活动时间、术后住院时间、住院费用及满意度等指标上均优于对照组,差异有统计学意义（P〈0.05）,在术后并发症上两组差异无统计学意义（P＆gt;0.05）.结论 舒适化医疗理念应用于胃癌患者围术期能够明显提高患者的舒适化体验,加快术后康复,并且安全有效.Objective To explore the application effect of comfortable medical concept in patients with gastric cancer surgery.Methods A total of 238 patients who underwent radical resections for gastric cancer in Department of Gastrointestinal Surgery were chosen from January 2015 to November 2015. These patients were divided into observation group （120 cases） and control group （118 cases） according to development time of comfortable medical concept. Comfortable medical concept was applied in the observation group and routine medical concept for control group. Preoperative anxiety score, postoperative pain score, postoperative length of intestinal function recovery time, leaving bed and hospital stay, hospitalization expense, postoperative complication and satisfaction degree were observed as the main indexes to discriminate the two groups.Results Patients in observation group were demonstrated having lower postoperative anxiety score and pain score, shorter length of postoperative intestinal function recovery time and hospital stay, faster time of leaving bed, lower hospitalization expense, higher satisfaction degree compared with that of control group （P〈0.05）. The rate of postoperative complications had no statistical difference between two groups （P〉0.05）.Conclusions It is safe and effective to apply the comfortable medical concept to perioperative treatments for gastric cancer patients, which can significantly improve the patient′s comfortable experience and accelerate the postoperative rehabilitation.
