摘  要:目的评价高分辨黑血磁共振成像技术(BB-MRI)对蛛网膜颗粒定量评价的准确性。方法前瞻性纳入经时间飞跃法磁共振静脉造影(TOF-MRV)诊断存在蛛网膜颗粒的25名患者,进行BB-MRI扫描。对于每个测量对象(蛛网膜颗粒),在TOF-MRV图像上选取蛛网膜颗粒最大层面,将BB-MRI图像与之进行配准,获得重建图像。由两名有经验的放射科医师分别对TOF-MRV和BB-MRI图像上蛛网膜颗粒最大径线和最大截面积行双盲测定,比较两种方法测量指标的差异。结果在25名患者中,TOF-MRV共发现26个蛛网膜颗粒,BB-MRI方法均能正确检出。TOF-MRV和BB-MRI测定的蛛网膜颗粒最大径线平均值分别为(7.1±2.2)mm和(7.2±2.1)mm(P>0.05),最大截面积平均值分别为(25.3±8.8)mm^2和(25.4±8.8)mm^2(P>0.05)。BlandAltman分析显示蛛网膜颗粒最大径线、最大截面积的测量,两种方法具有良好的一致性。结论 TOF-MRV和BB-MRI技术均能无创性直观显示蛛网膜颗粒,可用于蛛网膜颗粒和静脉窦相关疾病的临床鉴别诊断。Objective To explore the value of high resolution black blood magnetic resonance imaging(BB- MRI) in the quantitative evaluation of araehnoid granulations. Methods 25 eases of patients with arachnoid granulations confirmed by time of flight magnetic resonance venography(TOF-Mf/V) were prospectively enrolled. For each arachnoid granule, two experienced radiologists were selected to determine the maximum diameter and cross-sectional area by reconstructed TOF-MRV and BB-MRI images blindly. Results A total of 25 patients were recruited and 26 arachnoid granules were found by TOF-MRV and confirmed by BB-MRI further. The maximal diameter of the arachnoid granules measured by TOF-MRV and BB-MRI was (7.1 ±2.2)mm and (7.2 ±2.1)mm respectively, and the maximum cross-sectional area was (25.3 ± 8.8)mm^2 and (25.4 ± 8.8)mm^2, respectively(P〉0.05). The Bland-Altman analysis showed that the maximum diameter and maximum cross-sectional area by TOF-MRV and BB-MRI were well consistent. Conclusion TOF-MRV and BB-MRI can noninvasively depict araehnoid granule in the venous sinus, would be useful for the diagnosis and differential diagnosis of the diseases.
