经皮腔内血管成形术治疗糖尿病足的疗效分析

摘　　要：目的探究经皮腔内血管成形术在治疗糖尿病足血管病变中的疗效。方法选择2012年1月至2015年1月间108例糖尿病足患者,随机分为对照组54例和实验组54例。对照组采用常规疗法,实验组采用经皮腔内血管成形术,术后进行随访,评价指标包括主观指标(疼痛缓解程度和肢体发冷改善程度)、客观指标(踝肱指数ABI、溃疡愈合情况)、临床成功率、足背动脉直径、患肢皮温和术后1年内截肢率。结果实验组疼痛缓解程度和肢体发冷改善程度显著高于对照组(P<0.05);实验组ABI改善和溃疡愈合情况明显优于对照组(P<0.05);治疗后实验组背动脉直径、患肢皮温较对照组均有显著改善(P<0.05);实验组手术成功率较高、1年内患肢截肢率较低(P<0.05)。结论应用经皮腔内血管成形术治疗糖尿病足,手术成功率高、主客观指标均得到显著改善,长期随访截肢率低,值得推广。Objective To study the curative effect of percutaneous transluminal angioplasty （PTA） on the treatment of diabetic foot vascular disease. Methods 108 patients with diabetic foot from January 2012 to January 2015 were selected to be randomly divid- ed into control group and experimental group （54 cases in each group）. The control group was treated by routine treatment and the ex- perimental group by PTA. Through postoperative follow-up visit, the subjective indicators （degree of pain relief and cold limbs im- provement） and objective indicators （ankle-brachial indexes, ulcer healing condition）, clinical success rate, diameter of dorsal pc- dis artery, skin temperature of the limb and amputation rate in 1 year after surgery were assessed. Results The degree of pain reliefand cold limbs improvement of experimental group was significantly higher than that of control group （ P〈0. 05 ）. The improvement of ABI and ulcer healing condition of experimental group was obviously better than that of control group （ P〈0.05 ）. Diameter of dorsal pedis artery and skin temperature of the limb of experimental group was obviously improved compared with control group （ P〈0.05 ）. The success rate of experimental group was higher and the amputation rate in 1 year after surgery was lower （ P〈0. 05 ）. Conclusion Application of percutaneous transluminal angioplasty in treatment of diabetic foot has a higher success rate, obvious improvement of subjective and objective indicators, and lower amputation rate through long-term follow-up visit, which is worthy of popularization.
