摘 要:目的:探讨老年冠心病(CHD)患者冠脉病变严重程度与血清尿酸(UA)、同型半胱氨酸(Hcy)水平的相关性。方法:选择我院心内科于2014年1月至2015年12月收治的老年CHD患者98例。依据Gensini积分的不同,患者被分为低分组(≤30分,32例)、中分组(30~60分,33例)和高分组(≥60分,33例)。检测比较三组的血清UA、Hcy浓度,用Gensini积分法、Sullivan积分法分别计算患者的冠脉狭窄程度、冠脉内膜粥样斑块比例。结果:与Gensini低分组比较,中分、高分组的血清UA[(207.51±79.57)μmol/L比(324.12±81.36)μmol/L比(454.32±99.15)μmol/L]和Hcy[(10.32±6.84)μmol/L比(19.95±7.69)μmol/L比(28.92±7.85)μmol/L]水平均显著升高,且高分组的显著高于中分组的,P<0.05或<0.01。Spearman等级相关分析显示,CHD患者血清UA浓度与血清Hcy浓度呈显著正相关(r=0.557,P=0.042);Gensini积分、Sullivan积分分别与血清UA、Hcy浓度呈显著正相关(r=0.559~0.894,P<0.05或<0.01)。结论:老年冠心病患者冠脉病变的严重程度与血清UA、Hcy的浓度呈显著正相关,且联合检测效果可能更好,值得推广。Objective: To explore correlation among serum uric acid (UA), homocysteine (Hey) levels and coronary disease severity in aged patients with coronary heart disease (CHD). Methods: A total of 98 aged CHD patients, who were treated in our department from Jan 2014 to Dec 2015, were selected. According to Gensini score, they were divided into low score group (430 scores, n = 32), medium score group (30-60 scores, n = 33) and high score group (≥60 scores, n = 33). Serum concentrations of UA and Hey, coronary stenosis degree calculated by Gensini score and coronary intimal atherosclerotic plaque proportion calculated by Sullivan score were measured and compared among three groups. Results: Compared with low Gensini score group, there were significant rise in serum concentrations of UA[(207. 51 ± 79.57)μmol/L vs. (324. 12 ± 81.36) μmol/L vs. (454. 32 ± 99. 15) μmol/L] and Hey [ (10.32 ± 6. 84)μmol/L vs. (19.95 ± 7.69)μmol/L vs. (28.92 ± 7.85)μmol/L] in medium score group and high score group, and those of high score group were significantly higher than those of medium score group, P〈0.05 or d0.01. Spearman rank correlation analysis indicated that serum UA concentration was significant positively correlated with serum Hey concentration (r = 0. 557, P = 0. 042) ; Gensini score and Sullivan score were significantly positive correlated with serum concentrations of UA and Hey in CHD patients (r = 0. 559-0. 894, P〈0.05 or 〈0.01). Conclusion: Coronary disease severity is significant positively correlated with serum concentrations of UA and Hey in aged CHD patients. Their combined detection may have better effect, which is worth extending.
