摘  要:目的探讨雷公藤提取物联合α酮酸对糖尿病肾病患者血清同型半胱氨酸及血管内皮生长因子水平的影响。方法选取我院肾内科收治的糖尿病肾病患者186例,按随机数字表法分组,对照组93例予以常规治疗联合雷公藤多苷治疗,研究组93例在对照组基础上予以α酮酸治疗,观察并记录两组间肾脏功能、病情进展相关指标、氧化应激指标水平,同时对比临床疗效及不良反应发生率。结果对照组治疗有效率(80.65%)低于研究组治疗有效率(91.39%),差异有统计学意义(P<0.05);与对照组比较,研究组治疗后Scr、BUN、Cys C及24h尿蛋白水平较低,GFR水平较高,治疗后血清Hcy、VEGF及TGF-β1水平较低,HGF水平较高,差异有统计学意义(P<0.05);两组不良反应的发生率差异无统计学意义(P>0.05)。结论雷公藤提取物联合α酮酸能明显降低糖尿病肾病患者血清Hcy及VEGF水平,抑制肾间质纤维化,改善肾功能,临床疗效及安全性较高。Objective To explore the impact study of tripterygium extract combined with alpha acid on serum homocysteine and vascular endothelial growth factor in patients with diabetic nephropathy. Methods 186 patients with diabetic nephropathy treated with renal internal medicine were selected and grouped according to the random number table method. 93 cases in the control group were treated with conventional treatment combined with tripterygium polyglycosides. The 93 patients in the study group were treated with alpha ketoacid based on the control group. The renal function, progress correlation index, oxidative stress index level of the two groups were observed and recorded. At the same time, the clinical efficacy and the incidence of adverse reactions were compared. Results The effective treatment rate (80.65%) was lower than that in the study group (91.39%), which was statistically significant (P 〈 0.05). After treatment, the Scr, BUN, CysC and 24h urine protein level were lower, GFR level was higher, serum Hcy, VEGF and TGF- 13 1 level were lower, HGF level was higher compared with the control group, and the difference was statistically significant (P 〈 0.05). There was no significant difference in the incidence of adverse reactions (P 〉 0.05). Conclusion Tripterygium extract combined with alpha acid can significantly reduce serum Hcy and VEGF levels in patients with diabetic nephropathy, inhibit renal interstitial fibrosis, improve renal function, and have high clinical efficacy and safety.
