摘 要:目的探讨个案管理护理模式对冠脉搭桥患者自我管理能力及健康行为的影响。方法将40例冠脉搭桥患者分为研究组和对照组各20例,对照组患者采用常规护理,研究组在对照组的基础上行个案管理,两组患者以冠心病自我管理量表和健康行为表来评价干预前后两组患者的自我护理能力及健康行为改善情况。结果两组患者入院时的一般资料及病程无统计学意义(P>0.05),干预一年后,研究组在健康行为各维度总分均明显高于对照组,差异有统计学意义(P<0.05),研究组的健康教育知晓率、满意度、治疗依从性、冠心病自我管理能力均高于对照组,两组比较差异有统计学意义(P<0.05)。结论个案管理护理能提高冠脉搭桥患者自我管理能力,改善患者健康行为。Objective To explore the effect of case management nursing model on self-management ability and health behavior of patients with coronary artery bypass grafting. Methods Forty patients with coronary artery bypass were divided into study group(20 cases) and control group(20 cases). The patients in the control group were treated with routine nursing. The patients in study group was treated with ease management on the basis of the control group. The self-care ability and healthy behavior improvement of patients in two groups were evaluated before and after the intervention with coronary heart disease self-management scale and health behavior table. Results The general data and course of the two groups were not statistically significant (P〈 0.05). After one year of intervention, the total score of SF-36 in the study group was significantly higher than that in the control group, the difference was statistically significant (P 〈 0.05). The health education awareness, satisfaction, treatment compliance and self-management ability of coronary heart disease of patients in the studygroup were higher than that in the control group (P 〈 0.05). Conclusion Case management nursing can improve the self-management ability of patients with coronary artery bypass grafting and improve the health behavior of patients.
