山豆根中毒致儿童中枢神经系统损害10例

摘　　要：目的分析山豆根中毒致儿童中枢神经系统损害的临床特点及转归,为临床诊治提供参考。方法 2012年6月至2016年5月,第四军医大学第一附属西京医院儿科共收治10例山豆根中毒患儿,均每日服用含山豆根的中草药1付。回顾性分析患儿的临床表现、头颅MRI结果、治疗方法及转归。结果 3例首发消化道症状,7例首发症状为头晕头痛。10例患儿均出现神经系统表现,包括步态不稳、四肢无力、流涎、失语、抽搐、视物不清、意识障碍。9例行头颅MRI检查,3例正常;6例发现小脑病变。予洗胃、补液、利尿、改善代谢、脱水、营养神经等方法治疗后,9例患儿均临床治愈出院,1例因呼吸衰竭放弃治疗后死亡。随访至2016年5月,8例患儿无任何神经系统后遗症,1例失访。结论山豆根中毒对儿童危害较大,主要表现为中枢神经系统损害,应注意防范,发病后及时诊治。Objective To analyze the clinical characteristics and outcomes of central nervous system damage in children with Radix Sophorae Subprostratae poisoning, and to provide reference for clinical diagnosis and treatment. Methods From June 2012 to May 2016, 10 children with Radix Sophorae Subprostratae poisoning were admitted to the pediatric department of Xijing Hospital of the Fourth Military Medical University. Clinical manifestations, head MRI results, treatment and outcomes were analyzed retrospectively. Results The first symptoms of three cases was gastrointestinal symptoms, and that of seven cases were dizziness or headache. Ten cases all showed neurological manifestations, including lower extremity ataxia, limb weakness, salivation, aphasia, convulsions, blurred vision and disturbance of consciousness. Nine cases underwent head MRI examination. The results of three cases were normal, and six cases were found cerebellar lesions. After gastric lavage, rehydration, diuretic, improving metabolism, dehydration, neurotrophic and other treatments, nine cases were clinically cured and discharged, but one case with respiratory failure was given up and died. Followed up until May 2016, eight cases showed no neurological sequelaes, and one case was lost to followed-up. Conclusions Radix Sophorae Subprostratae poisoning was harmful to children. Central nervous system damage was the main manifestation. Prevention must be paid attention. Diagnosis and treatment must be performed if poisoning.
