代谢综合征患者非心脏手术围术期心脏并发症的影响因素分析

摘　　要：目的探讨代谢综合征患者非心脏手术围术期心脏并发症的影响因素,为术后并发症的预防提供参考。方法选择2012年1月~2015年12月我院收治的265例代谢综合征非心脏手术患者作为研究对象,观察统计非心脏手术围术期患者的心脏并发症发生率。并将患者分为并发症组和无并发症组,观察两组患者临床资料、手术进行情况等因素对并发症发生的影响,采用Logistic回归分析对相关影响因素进行分析对比,探讨并发症发生的潜在危险因素。结果 265例手术患者围术期有30例患者出现并发症,发生率为11.32%;出现的并发症主要包括心律失常(11例)、心绞痛(9例)、心肌梗死(6例)、低心排综合征(4例)。经Logistic多因素回归分析显示,患者的年龄、手术时间、手术类型、心功能分级是引起代谢综合征患者非心脏手术围术期心脏并发症的主要危险因素。结论代谢综合征患者非心脏手术围术期并发症的发生与患者的年龄、手术方式选择、手术时间、患者心功能有密切关系。患者年龄轻、选择微创手术方式、缩短手术时间以及心功能较好的患者可降低术后并发症的发生率,也为临床上并发症的预防提供参考。Objective To explore the influencing factors of cardiac complications during non-cardiac surgery periopera- tire period in patients with metabolic syndrome,and to provide reference for the prevention of postoperative complica- tions.Methods From January 2012 to December 2015,265 patients with metabolic syndrome but without cardiac surg- eries admitted into our hospital were selected as research objects.The incidence rate of cardiac complications during non-cardiac surgery perioperative period was observed and calculated,they were divided into the complication group and the non-complication group,the influence of clinical data,operation condition and other factors on complications were observed in th two groups.Logistic regression analysis was used to analyze the related influencing factors,and to explore the potential risk factors of complications.Results Complications of 30 cases occurred in 265 patients during the perioperative period,and the incidence rate was 11.32%.The principal complications were arrhythmia （11 cases）, angina pectoris （9 eases）,myoeardial infarction（6 cases）,and low cardiac output syndrome （4 cases）.Logistie multivariate regression analysis displayed age,operatlon time and type,and grade of cardiac function were the main risk factors of cardiac complications during non-cardiac surgery perioperative period in patients with metabolic syndrome.Conclusion The incidence of complications during non-cardiac surgery perioperative period in patients with metabolic syndrome is closely related with patient＇s age,operation method,surgical time,and grade of Cardiac function.Young age,minimal-inva- sion surgery,shortened surgical time,and great cardiac function can decrease the incidence of cardiac function and also provide reference for the prevention of cardiac complications.
