PCT及IFN-γ在卒中相关性肺炎中的水平变化及诊断价值

摘　　要：目的探讨血清降钙素原(PCT)及γ-干扰素(IFN-γ)在卒中相关性肺炎(SAP)中的早期诊断价值。方法分别采取SAP及社区获得性肺炎(CAP)患者使用抗菌药物治疗前后静脉血及卒中未合并肺炎患者静脉血,采用双抗体夹心法酶联免疫吸附试验法检测各组血清PCT及IFN-γ水平。结果 SAP与CAP患者在起病初即表现有明显的PCT增高,但SAP与CAP组间比较无统计学差异,抗感染治疗7 d后,两组PCT水平均可明显下降;卒中未合并肺炎患者IFN-γ水平与健康对照者比较无统计学差异,SAP患者发病早期血清IFN-γ水平明显下降,治疗后IFN-γ水平明显上升,而CAP患者治疗前后IFN-γ比较均无统计学差异。结论 PCT及IFN-γ两者联合检测能从感染程度、免疫水平等角度提高SAP诊断的敏感性和特异性。Objective To evaluate the diagnostic value of serum procalcitonin（PCT） and Interferon-γ（IFN-γ） level changes in the early stage of stroke-associated pneumonia. Methods Sera were extracted from the patients with stroke-associated pneumonia（SAP） and community-acquired pneumonia（CAP） respectively before and after antibiotic treatment,and from stroke patients without pneumonia complication. Serum concentration of PCT and IFN-γ were detected by Sandwich enzyme-linked immuno-sorbent assay. Results At the early stage of SAP and CAP,the level of PCT exhibited an increasing trend,but there was no significant difference between SAP and CAP group. After 7 days antibiotic treatment,the levels of PCT all declined apparently in SAP and CAP group. There was no significant difference of IFN-γ between stroke patients without pneumonia complication and the healthy control objects. In the early stage of SAP,the level ＂of IFN-γ was very low,which was increased apparently after treatment,whereas,the level of IFN-γ didn＇t change by the treatment in CAP group. Conclusion Together detecting PCT and IFN-γ could improve the sensitivity and speciality in diagnosing SAP from the aspects of infectious degree and immune response.
