胃大部切除术治疗胃十二指肠溃疡大出血的临床效果

摘　　要：目的探讨胃大部切除术治疗胃十二指肠溃疡大出血的临床效果。方法按照手术方案不同将84例胃十二指肠溃疡大出血患者分为观察组(胃大部切除术)44例和对照组(单纯修补术)40例,比较两组患者治疗效果。结果两组患者治疗总有效率对比差异未见统计学意义(P>0.05);观察组手术时间和住院时间均显著长于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率显著高于对照组,但复发率显著低于对照组,差异有统计学意义(P<0.05)。结论胃大部切除术治疗胃十二指肠溃疡大出血效果显著,复发率低,但操作更为复杂,术后并发症发生率也更高,与单纯修补术各有优劣。Objective To investigate the clinical effect of subtotal gastrectomy on patients with massive hemorrhage of gastroduodenal ulcer. Methods A total of 84 patients with massive hemorrhage of gastroduodenal ulcer were randomly di- vided into observation group（ subtotal gastrectomy, 44 cases） and control group（ simply neoplasty, 40 cases）. The clinical effects were observed. Results There was no significant difference in the total effective rate between the two groups （P 〉 0. 05 ）. The time of operation and hospital stays in the observation group were higher than those in the control group （ P 〈0. 05）. The incidence of complication in the observation group was higher than that in the control group（P 〈0. 05）. The incidence of recurrence in the observation group was lower than that in the control group（ P 〈 0. 05 ）. Conclusions Subto- tal gastrectomy has remarkable effect on patients with massive hemorrhage of gastroduodenal ulcer, which has lower inci- dence of recurrence, complex operations, higher incidence of complication, has advantages and disadvantages.
