中药配合穴位按摩治疗早期乳痈

摘　　要：目的探讨中药配合穴位按摩方法治疗早期乳痈的临床疗效。方法 80例哺乳期急性乳腺炎初期患者,随机分为两组。治疗组40例给予中药瓜蒌牛蒡汤加减,日1剂口服,配合按揉膻中穴1min,按揉乳中穴、乳根穴2~5min,拇指、食指、中指轻捏拿乳头2min,五指从远端向乳头方向梳理乳房5min,每次治疗15~20min,每日1次,疗程5天;对照组40例给予头孢呋辛钠注射液0.75 g加入生理盐水250ml静滴,每8~12h一次,疗程5天。比较两组临床疗效。结果治疗组治愈28例,显效9例,有效3例,无效0例,总有效率为100.0%;对照组治愈19例,显效11例,有效7例,无效3例,总有效率为92.5%。两组临床疗效比较进行秩和检验后P<0.05,差异有统计学意义。结论中药配合穴位按摩及排乳手法治疗早期乳痈,疗效优于单纯抗生素治疗。Objective To observe the clinical effect of the therapy of TCM in the treatment of early mammary carbuncle. Method 80 cases of patients with early acute mammary carbuncle were randomly divided into the treatment group （ n = 40） and the control group （ n = 40）. The control group was treated with intravenous antibiotics alone. Specific methods of treatment were as follows ： intravenous drip of eefuroxime sodium 0.75g and saline 250ml repeated q8h-12h for 5 days as a course of treatment. The treatment group was given accupoint massage and prolactin massage besides traditional Chinese medicine decoction on oral Gualou Niubang decoction daily does. The specific methods of aceupoint massage and prolaetin massage were as follows： massaging the Danzhong （RN17） for one minute, massaging the Ruzhong （ST17） and Rugen （ST18） for 2 to 5minutes, massaging the nipple softly with the thumb and first two fingers for 2 minutes, stroking the breast from far to near the nipple with fine fingers for 15 to 20minutes. All of the therapies above were done once per day for 5 days as a course of treatment. The clinical efficacy of the two groups was compared. Result In the treatment group, 28 cases were cured, 5 cases were excellence effective, 3 cases were effective, 0 was invalid, and the total effective rate was 100%. In the control group, 10 cases were cured, 12 cases were excellence effective, 7 cases were effective, 2 cases were invalid, and the total effective rate was 95%. Comparing clinical effect of two groups by rank sum testing, the difference was statistically significant （P 〈 0. 05）. Conclusion Chinese medicine combined with accupoint massage and prolactin massage in the treatment of early mammary carbuncle was more effective than treatment with antibiotics alone.
