集束化护理在老年危重患者人工气道管理中的应用

摘　　要：目的:探讨集束化护理在清除老年危重患者人工气道分泌物,预防肺部感染发生及气管导管痰痂堵管,缩短气道护理工时的应用效果。方法:2015年10月~2016年9月入住ICU建立人工气道老年危重患者128例,随机等分成试验组和对照组,试验组应用集束化气道护理方法;对照组应用传统气道护理方法,比较两组肺部感染、痰痂堵管、气道护理工时及入住ICU时间。结果:两组肺部感染,痰痂堵管及气道护理工时比较差异均有统计学意义(P<0.05),两组入住ICU时间无统计学意义(P>0.05)。结论:集束化气道护理老年危重患者人工气道能加强气道湿化,有效清除气道分泌物,减少气管导管痰痂堵管的发生,阻止细菌黏附,降低肺部感染率,缩短气道护理工时。Objective: To explore the effects of cluster nursing in the removal of artificial airway secretions in critically ill patients with old age and to prevent the occurrence of pulmonary infection and tracheal catheters as well as to shorten the application of airway care. Methods: A total of 128 critical elderly patients admitted in ICU to establish article airway from October 2015 to September 2016 were selected and they were randomly divided into experimental group and control group. The experimental group was treated with intensive airway care. The control group was treated with traditional airway nursing. Compared the two groups in lung infection,sputum scrapping,airway care hours and stay ICU time. Results: There was significant difference between the two groups in lung infection,phlegm obstruction in tube and airway care( P < 0. 05). There was no significant difference between the two groups in ICU hospitalization time( P > 0. 05). Conclusion: The artificial airway can strengthen the airway humidification,effectively remove the airway secretions,reduce the occurrence of tracheal tube sputum scrapping,prevent bacterial adhesion,reduce lung infection rate and shorten the airway nursing hours.
