经口气管插管病人口腔护理集束化策略的形成及其应用

摘　　要：[目的]探讨集束化策略在经口气管插管病人口腔护理中的应用效果。[方法]采用行动研究法对20例经口气管插管病人进行集束化策略的预试验;选取2014年10月—2015年3月我院ICU收治的140例次经口气管插管且行常规口腔护理的病人作为对照组,选取2015年5月—9月我院ICU收治的140例次进行经口气管插管病人集束化口腔护理作为干预组,采用口臭、口腔污染度及牙菌斑指数来评价病人的口腔状况,并通过护士访谈评价可行性,依据不安全事件发生率评价安全性。[结果]干预组口臭、口腔污染度和牙菌斑指数评分均低于对照组,差异均有统计学意义(P<0.05);多数护士对集束化策略的可行性给予了肯定;仅有2例病人发生误吸呛咳现象。[结论]口腔护理集束化策略的应用,能有效改善经口气管插管病人的口腔卫生状况,并一定程度上提高ICU护士对危重症病人口腔护理的重视程度。Objective:To explore the effect of cluster strategy for oral care for patients with oral tracheal intubation.Methods:The preoperative trial of 20 patients with oral tracheal intubation was performed by action research.A total of 140 cases of patients with oral tracheal intubation and routine oral care treated by ICU in our hospital from October 2014 to March 2015 were selected as control group.From May to September 2015,a total of 140 patients with oral tracheal intubation receiving cluster oral care and treated in ICU were selected as the intervention group.The patients were assessed the oral hygiene condition from aspects of bad breath,oral pollution degree and dental plaque index.The feasibility was assessed through the nurses' interview,and the safety was assessed by the incidence of unsafe incidents.Results:The scores of bad breath,oral pollution degree and dental plaque index in intervention group were lower than those in control group(P <0.05).Most nurses gave a certainty to the feasibility of the cluster strategy.Only two patients had aspiration and cough.Conclusions:The application of cluster strategy for oral care could improve the oral hygiene condition of patients with oral tracheal intubation and improve the attention degree of ICU nurses' to the oral care of critically ill patients.
