摘　　要：[目的]探讨TIME-H量表在慢性伤口治疗评价中的不同层级评定者间信度可靠性研究。[方法]采用方便取样的方法,选取30例慢性伤口病人,由6名不同层级的伤口专科护士应用TIME-H量表分别对30例慢性伤口的愈合效果进行独立评价。应用组内相关系数(ICC)对评定者间信度进行评价。[结果]TIME-H量表中伤口局部评估中控制感染项ICC值最低为0.923(95%CI0.872~0.959),清除坏死组织项ICC值最高为0.989(95%CI 0.981~0.994),TIME总分ICC值为0.990(95%CI 0.983~0.994);影响伤口愈合因素评估结果中共有6项,其中精神状况、营养、年龄的ICC值为1,易感染疾病ICC值最低为0.694(95%CI 0.487~0.837),总分ICC值为0.988(95%CI 0.979~0.993);TIME-H量表总分ICC值为0.991(95%CI 0.985~0.995),差异均具有统计学意义(P<0.05)。[结论]TIME-H评分在慢性伤口治疗评价中,不同层级评定者间信度可靠,提示该量表可作为临床伤口专科护士评价慢性伤口愈合情况和治疗效果的工具。Objective:To discuss the inter-rater reliability of TIME-H scale between different grades of raters in the evaluation of chronic wound treatment.Methods:A total of 30 patients with chronic wound were selected by convenient sampling method.Using the TIME-H scale,the healing effect of 30 cases was evaluated by six different grades of wound specialist nurses independently.The intra-group correlation coefficient(ICC)was used to evaluate the inter-rater reliability.Results:The lowest ICC value of control infection in the local wound assessment in the TIME-H scale was 0.923(95%CI 0.872~0.959).The highest ICC value of the remove necrotic tissue was 0.989(95%CI 0.981~0.994).The total ICC value of TIME was 0.990(95%CI0.983~0.994).There were 6items affecting the evaluation of wound healing factors,in which the ICC value of mental status,nutrition,and age were 1,the lowest ICC value of susceptible disease was 0.694(95%CI0.487~0.837),the total ICC value was0.988(95%CI0.979~0.993).The total ICC value of TIME-H scale was 0.991(95%CI 0.985~0.995),and the difference was statistically significant(P<0.05).Conclusions:The inter-rater reliability of TIME-H scale in the measurement of chronic wound between different grades of raters was good,suggesting that the scale could be used as an evaluating tool for evaluating healing condition and treatment effect of the chronic wound for clinical wound specialist nurses.
