摘 要:目的探讨彩色多普勒超声对多发性大动脉炎肾动脉狭窄的诊断与鉴别诊断价值。方法对经CT血管造影成像或数字减影血管造影证实的22例多发性大动脉炎肾动脉狭的声像图表现进行回顾性分析。总结其声像图特征。结果彩色多普勒超声可清晰显示多发性大动脉炎肾动脉狭窄的累及范围、狭窄处及以远血管内径和形态、管壁厚度及回声、有无五彩镶嵌血流、血流速度及频谱特征等信息。多发性大动脉炎肾动脉狭窄声像图表现为肾动脉起始部或近段的管壁增厚、管腔狭窄、流速增快,有别于动脉粥样硬化及纤维肌发育不良等其他导致肾动脉狭窄的疾病。结论彩色多普勒超声是诊断与鉴别诊断多发性大动脉炎肾动脉狭窄有价值的影像学方法。Objective To evaluate the value of color doppler ultrasonography in diagnosis and differential diagnosis of renal arterial stenosis of Takayasu arteritis. Methods The ultrasonographic images of twenty-two patients with Takayasu arteritis confirmed by CT angiography or digital subtraction angiography were retrospectively analyzed. The ultrasonographic features were summarized. Results The ultrasonographic images showed the involvement, diameter and shape, wall thickness and echo, multicolored blood flow, blood flow velocity and frequency spectrum characteristics of the stenosis and beyond clearly. Renal arterial stenosis of Takayasu arteritis showed vascular wall thickening, luminal stenosis and higher speed at origin or proximal part of renal artery, which was different from atherosclerosis and fibromuscular dysplasia and other causes of renal arterial stenosis. Conclusion Color doppler ultrasonography is the effective imaging method for diagnosis and differential diagnosis of renal arterial stenosis of Takayasu arteritis.
