摘  要:目的评价T_1WI、T_2WI、TIRM-T_2WI及3D-MEDIC-WE序列对骶髂关节炎关节软骨病变的诊断价值。方法收集临床高度怀疑骶髂关节炎的患者60例,另选取20例志愿者作为对照组。在3D-MEDIC-WE序列上测量对照组骶髂关节软骨厚度;在TIRM-T_2WI、3D-MEDIC-WE序列上分别对患者组骶髂关节软骨进行分级评分。结果正常骶髂关节软骨厚约(2.70±0.41)mm,骶侧软骨厚约(1.21±0.22)mm,髂侧软骨厚约(0.72±0.31)mm,骶侧软骨比髂侧软骨明显厚、约为其2倍(P<0.01)。患者组在3D-MEDIC-WE序列上的软骨侵蚀分数高于TIRM-T_2WI序列(P<0.01)。结论3D-MEDIC-WE序列可以清晰显示骶髂关节软骨;3D-MEDIC-WE序列对骶髂关节炎关节软骨病变的显示优于其他序列。Objective To evaluate the diagnostic value of MR T1WI, TIRM-T2 WI and 3 D-MEDIC-WE sequences for sacroili- ac joint (SIJ) cartilage erosion. Methods A total of 60 patients highly suspected as sacroiliitis were collected, and twenty healthy volunteers were enrolled into the control group; and four MRI sequences, including oblique-coronal-T1WI, oblique-coronal-T2 WI, oblique-coronal-TIRM-T2 WI and oblique-coronal-3D-MEDIC-WE were performed. The total thickness of SIJ cartilage, the cartilage of iliac and sacral sides in control group were measured respectively. The grading of SIJ cartilage in patient group were assessed according to predefined scale. Results The SIJ cartilage was (2.70 ± 0.41 ) mm, cartilage of sacrum ( 1.21 ± 0. 22 ) mm was thicker than that of ilium' s (0.72 ± 0.31 ) mm, and there was significant difference ( P 〈 0.01 ). Scores of cartilage erosion in 3D-MEDIC-WE sequence were statistiely higher than those in TIRM-T2WI sequences ( P 〈 0.01 ). Conclusion 3D-MEDIC-WE sequence ean identify SIJ cartilage, including sacral and iliac side and the gap between them. 3D-MEDIC-WE can clearly show SIJ cartilage and the diagnostic accuracy of 3D-MEDIC-WE sequence for cartilage injury is better than that of T1 WI, T2 WI, T2 WI-TIRM.
