应用新型可调节式骨盆外固定架治疗不稳定骨盆骨折的疗效观察

摘　　要：背景:不稳定骨盆骨折创伤严重,分型复杂,治疗方式多种多样。目的:观察新型可调节式骨盆外固定架在不稳定骨盆骨折急诊急救、临床治疗中的疗效。方法:2008年1月至2016年6月共收治64例不稳定骨盆骨折患者,根据骨盆骨折的Tile分型、严重程度及是否合并其他脏器损伤选择性应用新型可调节式骨盆外固定架治疗,并对其疗效进行观察。结果:全部患者均一次顺利完成外固定手术,11例患者二期行切开复位内固定加外固定术。术后全部病例完成随访,平均15个月,外固定架没有出现变形、折断等现象,无钉道感染。所有患者全部骨性愈合,优良率90.6%。结论:我院自行设计的可调节式骨盆外固定架是一种新型支架,在治疗不稳定骨盆骨折时简单、安全、微创、有效,可以在急诊室、病床边局麻下实施,能够迅速稳定骨盆环。根据骨盆骨折类型,既可以做为急诊抢救的临时固定装置,又可以某些不稳定骨盆骨折的终极治疗,还可以帮助骨盆骨折切开复位内固定获得理想复位。Background： Unstable pelvic fracture is a kind of severe injuries and has complicated typing. The treatment is varied. Objective： To observe the clinical outcome of new type adjustable pelvic external fixator for unstable pelvic fractures in first-aid situation and clinical practice. Methods： A total of 64 patients with unstable pelvic fractures were selected to receive external fixation for pelvis fractures using new type adjustable pelvic external fixator according to the Tile classification of pelvic fractures, severity and whether accompanying with other viscera damage, from January 2008 to June 2016. Results： All patients successfully received external fixation. Of them, 11 patients underwent two-stage operation for open reduction with internal fixation and external fixation. All patients completed a follow-up for an average of 15 months.During this period there was no such events as external fixator deformation or broken, or infection of pin track. All patients achieved bone union, and the excellent and good rate was up to 90.6%. Conclusions： Self-designed adjustable pelvic external fixator is a simple, safe, minimally invasive and effective new type of support in the treatment of unstable pelvic fractures. It can be used under local anesthesia in the emergency room and ward for quickly stabilizing pelvic ring. According to the type of pelvic fracture, it can be used not only as a temporary fixing device in emergency rescue but also as the final treatment for some unstable pelvic fractures, and help with the open reduction and internal fixation of pelvic fracture.
