磁共振弥散加权成像(DWI)、动态增强MRI(DCE-MRI)诊断乳腺良恶性病变价值对比研究  被引量:7

摘  要:目的探究磁共振弥散加权成像(DWI)、动态增强MRI(DCE-MRI)诊断乳腺良恶性病变的价值。方法随机选该院于2013年4月—2016年6月收治的怀疑为乳腺疾病的患者36例作为试验组,另外选取36名没有患乳腺疾病的健康者作为对照组,所有对象均进行磁共振弥散加权成像以及常规磁共振扫描,对试验组给予磁共振动态增强扫描,比较特异度、敏感度、阴性预测值以及阳性预测值。结果磁共振常规扫描对于诊断乳腺良恶性的特异度为57.1%,敏感度为81.8%,阴性预测值为66.7%,阳性预测值为75.0%;磁共振动态增强扫描对乳腺良恶性诊断的特异度为71.4%,敏感度为86.4%,阴性预测值为76.9%,阳性预测值为82.6%;动态增强扫描以及DWI联合使用对乳腺良恶性诊断的特异度为78.6%,敏感度为95.4%,阴性预测值为91.7%,阳性预测值为87.5%。结论动态增强MRI以及DWI联合使用能够明显提高诊断乳腺良恶性病变的阴性结果的估计值、阳性结果估计值、特异度以及敏感度。Objective To study the value of diffusion weighted imaging and dynamic contrast-enhanced MRI in diagno- sis of benign and malignant breast diseases. Methods 36 cases of patients suspected with breast diseases admitted and treated in our hospital from April 2013 to June 2016 were randomly' selected as the test group, and 36 cases of healthy persons without breast diseases were selected as the control group, and all patients were given the DWI and routine magnetic resonance imaging, while the test group adopted the DCE-MRI, and the specificity, sensitivity, negative pre- dictive value an positive predictive value were compared between the two groups. Results The specificity, sensitivity, negative predictive value an positive predictive value of routine magnetic resonance imaging, DCE-MRI and DWI and DCE-MRI in diagnosis of benign and malignant breast diseases were respectively 57.1%, 81.8%, 66.7%,-75.0% and 71.4%, 86.4%, 76.9%, 82.6% and 78.6%, 95.4%, 91.7%, 87.5%. Conclusion DWI and DCE-MRI can obviously im- prove the evaluation value of negative and positive results, specificity and sensitivity degree in diagnosis of benign and malignant breast diseases.
