洛匹那韦/利托那韦治疗梅毒合并HIV感染患者的CK-MB与LDH指标和CD_4^+T淋巴细胞水平及免疫相关指标的变化

摘　　要：目的研究梅毒合并人类免疫缺陷病毒(HIV)患者应用洛匹那韦/利托那韦治疗后的清肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)和外周血CD_4^+T淋巴细胞以及相关免疫指标水平的变化。方法选取2014年2月-2016年12月医院的梅毒合并HIV患者73例,按数字随机法分为对照组36例,研究组37例;对照组给予齐多夫定(AZT)+拉米夫定(3TC)+依非韦伦(EFV);研究组给予AZT+3TC+洛匹那韦/利托那韦(克力芝);比较两组患者临床指标。结果治疗后,研究组的CK-MB(0.71±0.02)mg/L、LDH(470.89±18.72)U/L比对照组CK-MB(1.16±0.52)mg/L、LDH(543.49±14.07)U/L下降显著(P<0.05);研究组的CD_4^+T计数(265.08±105.36)ng/L比对照组的CD_4^+T(178.64±110.52)ng/L指标显著回升(P<0.05);研究组免疫细胞学功能指标明显低于对照组,而血液免疫球蛋白含量优于照组(P<0.05)。结论洛匹那韦/利托那韦治疗梅毒合并HIV患者,能够使患者的CK-MB、LDH和CD_4^+水平显著改善,并且对相关的免疫功能有积极影响,联合抗病毒药物AZT和3TC治疗梅毒合并艾滋病具有更高效、更理想的治疗效果。OBJECTIVE To investigate the changes of creatine kinase isoenzyme MB subtype（CK-MB）,lactate dehydrogenase（LDH）and peripheral CD4＋T lymphocytes and related immune indexes of lopinavir/ritonavir in the treatment of patients of syphilis with HIV infection.METHODS A total of 73 patients diagnosed syphilis with HIV infection in our hospital from Feb.2014 to Dec.2016 were chosen.They were divided into two groups according to digital randomization and taken with different treatment regimens.The control group of 36 patients was taken with zidovudine（AZT）＋ lamivudine（3TC）＋ efavirenz（EFV）,and the study group was taken with zidovudine（AZT）＋lamivudine（3TC）＋lopinavir/ritonavir.The clinical indexes between the two groups were compared.RESULTS After treatment,the CK-MB（0.71 ± 0.02 mg/L）and LDH（470.89±18.72 U/L）in study group were significantly lower than（1.16±0.52 mg/L）and（543.49±14.07 U/L）in control group（P〈0.05）.The CD4＋T（265.08±105.36 ng/L）in study group was significantly higher than（178.64±110.52 ng/L）in control group（P〈0.05）.The immune function indexes in study group were significantly lower than those in control group（P〈0.05）.CONCLUSIONLopinavir/ritonavir in the treatment of syphilis with HIV can significantly improve the levels of CK-MB,LDH and CD4＋ in patients and have a positive effect on the associated immune function.Combined with antiviral drugs AZT and 3TC,it has more efficient and more ideal treatment effects in the treatment of syphilis with AIDS.
