乳腺手术后局部组织感染的革兰阳性菌特征及耐药性研究

摘　　要：目的分析引起乳腺手术后局部组织感染的革兰阳性菌特征及药敏试验结果,为乳腺手术后局部组织感染的治疗提供参考。方法选取2012年1月-2016年10月于医院行乳腺手术治疗术后发生局部组织感染的120例患者临床资料,患者在术后有局部组织感染迹象时采集局部分泌物,行细菌培养及药敏试验。结果 120例患者其中86例培养出革兰阳性菌,阳性率为71.67%,革兰阳性菌共97株,以葡萄球菌属为主,共75株占77.32%;葡萄球菌属对阿莫西林/克拉维酸、氨苄西林/舒巴坦、头孢哌酮、万古霉素耐药率均<50.00%,对环丙沙星、红霉素、庆大霉素、左氧氟沙星、呋喃妥因、苯唑西林钠、青霉素、四环素耐药率>50.00%。结论引起乳腺手术后局部组织感染的革兰阳性菌以葡萄球菌属为主,其次为肠球菌属,以单一病原菌感染多见,葡萄球菌属和肠球菌属对阿莫西林/克拉维酸、氨苄西林/舒巴坦、头孢哌酮和万古霉素等抗菌药物耐药率低,对其他临床常用的抗菌药物耐药性严重。OBJECTIVE To analyze the characteristics and drug susceptibility test results of gram-positive bacteria of local tissue infections after breast surgery,so as to provide the reference for the treatment of local tissue infections after breast surgery.METHODS METHODS The clinical data of 120 patients with local tissue infections after breast surgery from Jan.2012 to Oct.2016 were selected.The infected site secretions were collated after signs of local tissue infections.Bacterial culture and drug sensitivity test were performed.RESULTS Among 120 cases of patients,gram-positive bacteria were cultured in 86 cases,and the positive rate was 71.67% .Totally 97 strains gram-positive strains of bacteria were cultured,which were mainly 75 strains of Staphylococcus spp,accounting for 77.32% .The drug resistant rates of Staphylococcus sppto amoxicillin/clavulanic acid,ampicillin/sulbactam,cefoperazone and vancomycin were all lower than 50.00% ,and to ciprofloxacin,erythromycin,gentamicin,levofloxacin,furazolidone,oxacillin sodium,penicillin and tetracycline were all higher 50.00% .CONCLUSION Staphylococcus sppare the main gram-positive bacteria causing local tissue infections after breast surgery,followed by Enterococcus spp,and infections with a single pathogen are common.The resistance rates of Staphylococcus sppand Enterococcus sppto amoxicillin/clavulanic acid,cefoperazone/shubatan,ampicillin and vancomycin are low,and to other clinical commonly used antimicrobial are serious.
