摘 要:目的分析脊柱外科患者医院感染的病原菌分布及耐药性,为其预防和控制提供理论依据。方法回顾性分析2013年1月-2016年1月脊柱外科患者3 702例的临床资料,主要包括不同疾病的医院感染率、感染部位、感染菌株等;对发生医院感染患者的病原菌分布及耐药性进行检测。结果有36例患者发生医院感染,医院感染率为0.97%;感染部位以呼吸道为主,共21例占58.33%;共分离出病原菌154株,以革兰阴性菌为主,共106株占68.83%;铜绿假单胞菌、大肠埃希菌、肺炎克雷伯菌对氨苄西林耐药率较高,均>70.00%;金黄色葡萄球菌、表皮葡萄球菌对青霉素G、红霉素耐药率较高,均>80.00%。结论脊柱外科患者医院感染的病原菌分布及耐药性有一定的特点,临床医务人员根据药敏试验结果选取合理抗菌药物,能显著降低医院感染率、提高手术效果。OBJECTIVE To analyze the distribution and drug resistance of pathogens of nosocomial infections in patients with spinal surgery,so as to provide basis for its prevention and control.METHODS The clinical data of3702 patients with spinal surgery from Jan.2013 to Jan.2016 were retrospectively analyzed,including nosocomial infection rates,infection sites and infected strains,and the distribution and drug resistance of pathogens in patients with nosocomial infections were detected.RESULTS Totally 36 cases of patients occurred nosocomial infections,and the infection rate was 0.97% .The main infection site was 21 cases of respiratory tract,accouting for58.33% .Totally 154 strains of pathogens were isolated,which were mainly 106 strains of gram-negative bacteria,accounting for 68.83% .The resistant rates of Pseudomonas aeruginosa,Escherichia coli and Klebsiella pneumoniae to ampicillin were high,which were all higher than 70.00% .The resistant rates of Staphylococcus aureus and Staphylococcus epidermidis to penicillin G and erythromycin were all higher than 80.00% .CONCLUSION The distribution and drug resistance of pathogens in nosocomial infections in patients with spine surgery have some characteristics,and clinical medical personnel should select reasonable antibacterial drugs according to the result of drug sensitivity test,which can significantly reduce nosocomial infection rates and improve the operation effect.
