医护人员手与移动电话病原菌检出情况及对肿瘤科室患者医院感染的影响

摘　　要：目的探讨肿瘤科室医护人员手部与移动设备病原菌对医院感染的影响,为临床治疗提供参考依据。方法选取2014年1月-2016年8月医院肿瘤科32名医务人员进行调查研究,调查记录医护人员每天洗手次数,洗手方式;在接触患者前、接触患者后、接触体液后、接触患者环境后和无菌操作前,随机对医务人员进行手部及其所持移动电话(32部手机)进行采样,检出致病菌,统计医院感染率。结果护士和护生实施洗手率显著高于医师,差异有统计学意义(P<0.05);医护人员接触患者后、接触体液后、接触患者环境后手部暂居菌的含量显著上升(P<0.05),32部移动电话暂居菌的含量在不同时间段内比较差异无统计学意义;手部革兰阳性菌与革兰阴性菌分别占75.00%、25.00%,移动电话革兰阳性菌与革兰阴性菌分别占68.75%、31.25%;将医院感染与医护人员手消毒合格率进行直线相关分析,两者间具有负相关(r=-0.79,P=0.00)。结论肿瘤科医护人员手部及所持移动电话卫生状况不容乐观,手部和移动电话与医院感染存在相关性;应采取积极的措施,加强医护人员对手卫生的认识程度,降低或禁止移动操作状态时移动电话的使用。OBJECTIVE To investigate the influence of hand and mobile device pathogens on the nosocomial infections in oncology department,so as to provide references for clinical treatment.METHODS From Jan.2014 to Aug.2016,32 medical staff in oncology department were investigated.The number of daily hand washing and hand washing methods were recorded.The pathogens were sampled randomly from the hand and the mobile phone（32mobile phones）before and after contacting patients,after contacting with body fluids,after contacting with the patient＇s environment,and before the aseptic operation.The pathgens were detected,and the nosocomial infection rates were calculated.RESULTS The hand washing rate of nurses was significantly higher than those of doctors（P〈0.05）.After contacting with the patients,after contacting with the body fluid,and after contacting with the patient′s environmen,the number of temporary bacteria in hands were significantly increased（P〈0.05）.There were no significant changes in the content of temporary bacteria in 32 mobile phones at different time periods.Gram-positive bacteria and gram-negative bacteria in hand accounted for 75% and 25% ,respectively,and accounted for 68.75% and 31.25% in mobile phones,respectively.The linear correlation analysis between nosocomial infection and hand disinfection rate of medical staff was conducted,and there was a negative correlation between them（r=-0.79,P=0.00）.CONCLUSIONThe hygiene status of hand and mobile phone is not optimistic in medical staff of oncology department,and there is a correlation between hand and mobile phone and nosocomial infections.It should take positive measures to strengthen the awareness of medical staff on the hand hygiene,and reduce or prohibit the use of mobile phones when the mobile operating state.
