摘 要:目的观察3D打印双侧肩胛骨模型在先天性高肩胛症儿童治疗中的应用。方法从2011年4月至2016年2月我科治疗先天性高肩胛症儿童10例,男3例,女7例;左侧4例,右侧6例。按照Cavendish分级,Ⅱ级2例,Ⅲ级6例,Ⅳ级2例。8例儿童在术前行双侧肩胛骨3D打印模型,2例儿童行双侧肩胛骨并锁骨、颈椎、胸廓全打印。所有儿童均行Woodward手术治疗。结果 10例患者中6例术前患侧肩胛骨形态学有明显改变,4例大体形态正常。4例有肩椎骨。术后随访6个月至5年10个月。术后按照Cavendish分级,Ⅰ级6例,Ⅱ级4例。6例随访时再次进行3D打印模型,并与术前对比。肩胛骨的高度和宽度均增加,大体形态没有明显变化。结论 3D打印肩胛骨模型可以直观地显示患侧肩胛骨与正常侧的差别,有助于术前的手术设计。3D立体打印肩胛骨模型较单纯肩胛骨模型能在空间上直观地显示肩胛骨与周围骨骼的关系,对术前的手术设计更为有利。Objective To observe the application of 3Dprinting bilateral scapula model in the treatment of Sprengel deformity in children.Methods From April 2011 to February 2016,there were 10 cases of Sprengel deformity including3 boys and 7girls.The left side was affected in 4cases,right side was affected in 6cases.According to Cavendish classification,2cases were gradeⅡ,4cases were gradeⅢ,2cases were gradeⅣ.8children were underwent bilateral scapula 3Dprinting model.2children underwent bilateral scapula and clavicle,cervical vertebra,thoracic full print.All children underwent Woodward surgery.Results Preoperative ipsilateral scapula morphology changed obviously in 6cases.The shapes of affected scapula were regular with the contralateral sides in 4cases.4cases had omovertebral bones.All cases were followed from 6months to 5years 10 months.According to Cavendish classification,6cases were grade I,4cases were gradeⅡ postoperatively.6cases were performed 3Dprinting model again.The height and width of the scapula increased,but there was no significant change in the gross morphology.Conclusion 3D model can display the differences about the ipsilateral scapula and the normal side directly.It is helpful in the preoperative design.The 3Dprinting scapula model can display the relationship between the affected scapula and the surrounding bones directly in space,which is more helpful in the preoperative design.
