摘  要:目的探讨脊髓损伤后长期阳极阻滞电刺激在重建膀胱功能方面的作用。方法研究纳入新西兰兔60只,随机分为对照组(n=10)、脊髓损伤组(n=40,包含模型组及电刺激组)及解剖学观察组(n=10)。电刺激组兔于电刺激四周后,与对照组、模型组兔再次复查尿流动力学并进行组间比较,分析神经源性膀胱在脊髓损伤后所产生的尿动力学变化及膀胱功能在长期阳极阻滞电刺激后的情况。结果躯体运动神经在电刺激骶神经前根不断加大强度的同时逐渐被阻滞。长期阳极阻滞电刺激后,尿流动力学结果提示电刺激可减少膀胱的残余尿量同时提高膀胱顺应性而使得排尿体积增加,即提高了脊髓损伤后的排尿效率。结论融合骶神经根电刺激的阳极阻滞是一种重建脊髓损伤后膀胱排尿功能的有效方式。Objective To investigate the role of long-term anode block electrical stimulation in the reconstruction of bladder function after spinal cord injury.Methods A total of 60 New Zealand rabbits were randomly divided into control group(n=10),spinal cord injury group(n=40,including neurogenic bladder model and sacral nerve root stimulation group)and anatomical observation group(n=10).After four weeks of electrical stimulation,the urodynamic re-examination was performed with the electrical stimulation group,control group and the model group.The urodynamic changes of the neurogenic bladder after spinal cord injury and the improved conditions of the bladder function after long-term anode block electrical stimulation were analyzed.Results Somatic motor nerve in the stimulation of the anterior sacral nerve to increase the intensity and the same time gradually blocked.After long-term anode block of electrical stimulation,urodynamic results suggest that electrical stimulation can reduce the residual urine volume of the bladder while improving bladder compliance and urinary volume increased,that is,increased the efficiency of urinationafter spinal cord injury.Conclusion The coalesce of anode blocking technology and sacral nerve root stimulation method can effectively reconstruct bladder urination function after spinal cord injury.
