地佐辛复合舒芬太尼对结直肠癌术后镇痛的疗效分析

摘　　要：目的 探讨地佐辛复合舒芬太尼用于结直肠癌手术后患者静脉自控镇痛（PCIA）的疗效.方法 选择腹腔镜下结直肠癌手术患者60例,随机分为A组和B组,每组30例.手术完成前20 min,静脉给予舒芬太尼0.1 μg/kg,术后48 h给予PCIA,A组PCIA为地佐辛0.5 mg/kg＋舒芬太尼1.0 μg/kg,B组PCIA为舒芬太尼2.0 μg/kg,比较两组镇痛效果.结果 两组患者在观察期内均未出现血压过低和心动过缓的状况（P〉0.05）.A组在术后12、24 h血压、心率有所升高（P〉0.05）.术后VAS、Ramsay评分两组患者相同时间点比较,差异未见统计学意义（P〉0.05）. 术后不良反应发生率A组低于B组,差异有统计学意义（P〈0.05）.结论 地佐辛复合舒芬太尼应用于结直肠癌术后镇痛效果较好,有利于减少舒芬太尼的剂量,减少不良反应.Objective To investigate the effect of dezocine combined sufentanil on patient-controlled in intravenous analgesia （PCIA） after colorectal cancer surgery.Methods Sixty colorectal cancer patients underwent laparoscopic radical resection were selected, and they were randomly divided into group A and group B, with 30 cases in each group.Twenty minutes before the operation was completed, 0.1 μg/kg sufentanil was injected intravenously.PCIA was carried out 48 h after operation, group A was treated with 0.5 mg/kg dezocine＋1.0 μg/kg sufentanil, and group B was treated with 2.0 μg/kg sufentanil.The analgesic effects were compared between the two groups.Results Hypotension and bradycardia of the two groups were not fund during the observation period （P〉0.05）, blood pressure and heart rate in the group A increased 12, 24 h after operation（P〉0.05）.There were no significant differences in postoperative VAS, Ramsay score at same time point postoperative between the two groups （P〉0.05）.The incidence of adverse reactions in group A was less than that in group B, the difference was significant （P〈0.05）.Conclusions Dezocine combined with sufentanil has good analgesia effect on PCIA after colorectal cancer surgery, with fewer adverse reactions, which can reduce the dosage of sufentaniln.
