摘 要:目的探讨不同血清型乙型肝炎患者血清和唾液中HBV-DNA的差异以及相关性.方法选取60例乙型肝炎患者,分别采用实时荧光定量PCR法同时检测血清与唾液HBV-DNA,用ELISA检测血清HBV标志物,动力学方法检测血清ALT值,并分析其相互关系.结果60例乙型肝炎患者血清与唾液HBV-DNA阳性率分别为93.3%和63.3%(x2=15.91,P<0.01);HBeAg阳性患者33例,HBeAg阴性患者26例,其血清HBV-DNA阳性率分别为97.15%和88.5%(x2=0.64,P>0.05);ALT值的四分位间距分别为69.0(45.0~220.5)和38.5 (24.0~103.5)(t=1.36,P=0.180);血清HBV-DNA对数值的四分位间距分别为7.14(6.01~7.62)和5.09(2.30~6.83)(t=3.70,P<0.05);唾液HBV-DNA对数值的四分位间距分别为4.63(1.19~5.29)和0.00(0.00~4.6)(t=3.25,P<0.05).对60例乙型肝炎患者血清与唾液HBV-DNA病毒含量的对数值进行分析,发现两者呈显著的正相关(r=0.83,P<0.05),回归方程为y=0.88x+2.82.结论HBeAg阳性的患者与HBeAg阴性患者相比较,虽然血清中病毒阳性率以及ALT值无差异,但病毒含量有差异.乙型肝炎患者血清与唾液中的病毒含量呈正相关,血清中病毒含量高于唾液,提示血清的传染性比唾液强.Objective To investigate HBV-DNA contents in serum and saliva from hepatitis patients and the correlation with HBV serotypes.Methods Sixty hepatitis B patients were recruited.HBV-DNA in saliva and serum was tested by the real-time PCR method.HBV serotypes were identified by ELISA,and serum ALT was measured by the kinetic method.The relationship of these four values was also analyzed.Results The positive rates of HBV-DNA in serum and saliva were 93.3% and 63.3% (x2=15.91,P〈0.01) among the 60 patients,respectively.There were 33 HbeAg-positive patients and 26 HbeAg-negative patients in our studies and their serum HBV positive rates was 97.15% and 88.5% (x2=0.64,P〉0.05),respectively.The inter-quartile range of ALT value in serum was 69.0 (45.0~220.5),38.5 (24.0~103.5) (t=1.36,P=0.180),respectively.The log value of HBV-DNA in serum was 7.14 (6.01~7.62),5.09 (2.30~6.83) (t=3.70,P〈0.05),respectively.The log value of HBV-DNA in saliva was 4.63 (1.19~5.29),0.00 (0.00~4.60) (t=-3.25,P〈0.05).The log values of HBV-DNA in saliva and serum among 60 patients showed a positive relations (r=0.83,P〈0.05).The regression equation was y=0.88x+2.82.Conclnsions There were no significant differences among the positive rates and ALT in serums from HbeAg-positive patients and HbeAg-negtive patients,but the value of HBV-DNA was different.The positive relation between in serum and in salive was found.The viral loads was higher in serum than in saliva in HBV patients,and the infectivity of serum was higher saliva.
