老年高血压病患者平滑指数对阵发性心房颤动的影响

摘　　要：目的:探讨老年高血压病患者平滑指数与阵发性房颤(PAF)的相关性。方法:选取老年高血压病患者214例,根据有无PAF病史分为PAF组(71例)和窦性心律组(143例)。所有患者进行动态血压和超声心动图监测,观察血压变异性指标及心脏超声指标并进行组间比较和相关性分析。结果:PAF组患者的年龄、高血压病程、左房前后径、左室后壁均高于窦性心律组(均P<0.05),而三酰甘油和平滑指数均低于窦性心律组(均P<0.05)。多元线性逐步回归分析提示,左房前后径、左室后壁、平滑指数与PAF存在独立相关性,其中平滑指数的相关性最强(t=3.173,P=0.006)。结论:平滑指数与PAF有独立相关性,是评价老年高血压病患者PAF发生最简便的敏感指标。Objective：To explore the correlation between smooth index and paroxysmal atrial fibrillation（PAF）in elderly patients with hypertension.Method：A total of 214 elderly hypertensive patients were divided into PAF group（n=71）and sinus rhythm group（n=143）via the results of case history.Synchronic 24 hambulatory blood pressure monitoring and echocardiographic monitoring were performed for all participants.The comparison of variables between the two groups and correlation analysis were conducted.Result：The age,duration of hypertension,left atrial diameter,left ventricular posterior wall of PAF group were higher than those of sinus rhythm group（all P0.05）,while the TG and smooth index were lower than those of sinus rhythm group（both P0.05）.Multiple linear regression analysis showed that PAF was independently correlated with left atrial diameter,left ventricular posterior and smooth index,and the smoothness index was the strongest correlation（t=5.673,P=0.000）.Conclusion：The smaller smooth index may be an easy indicator to reflect the risk of PAF,and can forecast the PAF in elderly hypertensive patients.
