中药保留灌肠联合灸法治疗溃疡性结肠炎的随机对照研究

摘　　要：目的观察薏苡附子败酱散合芍药汤灌肠联合灸法治疗溃疡性结肠炎的临床疗效。方法选择溃疡性结肠炎患者92例,随机分为治疗组和对照组,每组46例。治疗组在常规治疗的基础上采用薏苡附子败酱散合芍药汤灌肠联合灸法治疗,对照组只接受常规治疗。观察两组患者治疗前后细胞因子水平、症状、大肠黏膜积分变化及临床疗效。结果治疗后治疗组在细胞因子[超敏C-反应蛋白(Hs-CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平、症候积分(腹痛、腹泻、脓血便)、大肠黏膜方面比较,差异有统计学意义(P<0.05);治疗组与对照组临床疗效总有效率分别为93.48%、80.43%,差异有统计学意义(P<0.05)。结论薏苡附子败酱散合芍药汤灌肠联合灸法能够有效地缓解溃疡性结肠炎患者临床症状及大肠黏膜病变,改善细胞因子水平,值得进一步推广及应用。Objective To observe the clinical therapeutic effects on ulcerative colitis treated with enema with yiyi fuzi baijiang san and shaoyao tang in combination with moxibustion.Methods Ninety-two patients of ulcerative colitis were randomized into a treatment group and a control group,46 cases in each one.In the treatment group,on the basis of the conventional treatment,the combined therapy of enema with yiyi fuzi baijiang san and shaoyao tang and moxibustion was adopted.In the treatment group,the conventional treatment was given.The cytokine levels,the score of symptoms and colon mucosa as well as clinical therapeutic effects were observed before and after treatment in the two groups.Results After treatment,the cytokines（Hs-CRP,IL-6 and TNF-alpha）,symptom scores（abdominal pain,diarrhea,pus and blood stool）,colon mucosa were different significantly as compared with those before treatment（P〈0.05）.The total effective rate in the treatment group was 93.48% and was 80.43% in the control group,indicating the significant difference（P〈0.05）.Conclusion The combined therapy of enema with yiyi fuzi baijiang san and shaoyao tang and moxibustion effectively relieves the clinical symptoms,colon mucosal disorders and improves in cytokine levels in the patients of ulcerative colitis.Hence,this therapy deserves to be further promoted.
