摘　　要：目的评价以急性腹痛为首发症状的睾丸扭转,以引起临床医生的高度重视。方法回顾性分析我院收治的31例睾丸扭转,其中1例睾丸扭转以急性腹痛为首发症状。结果 26岁男性患者因夜间突发转移性右下腹痛2h入院,经急诊室初步诊断为急性阑尾炎收入普通外科进行抗感染治疗,1d后出现右侧睾丸疼痛,行彩色多普勒超声发现右侧睾丸血流信号消失,转入泌尿外科行右侧睾丸切除术,左侧睾丸固定术。结论临床医生要警惕以腹痛为首发症状的睾丸扭转,彩色多普勒超声能准确判断睾丸扭转等急症,是睾丸扭转的首选检查,为了挽救睾丸,一旦确诊或者可疑睾丸扭转,有必要立即手术探查,手术探查是诊治睾丸扭转最可靠的方法。Objective To evaluate the diagnosis and treatment of testicular torsion with first symptoms of an acute abdomen pain. Methods Clinical data of 31 cases with testicular torsion were retrospectively reviewed, including a testicular torsion patient presenting as an acute abdomen pain. Results A 26 year old man presented as sudden transmited right low abdomen pain for 2 hours, and was first diagnosed as appendicitis. The patient complained of right testis pain on the second day. Color Doppler flow imaging （CDFI） confirmed testicular torsion. Subsequently the patients received right testicular resection and contralateral orchidopexy. Conehmion CDFI should be considered as a primary diagnostic approach for scrotal emergencies. Immediate operative exploration is done to improve the testicle survival rate once testicular torsion has been suspected. So it is necessary to perform orchidectomy and orchidopexy promptly under the condition of definite diagnosis of testicular torsion.
