自拟补肾生血方联合免疫抑制对成人慢性再生障碍性贫血患者T细胞亚群及造血调控因子的影响

摘　　要：目的 探讨自拟补肾生血方联合免疫抑制对成人慢性再生障碍性贫血（CAA）患者T细胞亚群及造血调控因子的影响.方法 选取CAA患者84例为研究对象,采用随机数字表法分为观察组和对照组,每组42例,对照组给予司坦唑醇片、环孢素软胶囊等西药治疗,观察组在此基础上加服自拟补肾生血方,两组均治疗6个月,比较两组患者的治疗有效率.应用酶联免疫双抗体夹心法监测两组治疗前后血清干扰素-γ（IFN-γ）、肿瘤坏死因子-α（TNF-α）的表达水平,并采用流式细胞仪测定两组治疗前后T细胞亚群,同时观察两组患者的不良反应.结果 观察组治疗有效率（92.86%）明显高于对照组（76.19%）（P〈0.05）;观察组治疗后IFN-γ、TNF-α、CD8＋水平低于对照组（P〈0.05）,观察组CD3＋、CD4＋、CD4＋/CD8＋水平高于对照组（P〈0.05）;两组不良反应发生率（11.90%比9.52%）比较差异未见统计学意义（P〉0.05）.结论 自拟补肾生血方联合免疫抑制可有效改善CAA患者T细胞亚群及造血调控因子,且不良反应轻,值得在临床推广应用.Objective To explore the effect of Bushen Shengxue decoction combined with immune suppression on T-cell subsets and hematopoiesis regulation factors of adult chronic aplastic anemia （CAA）.Methods Eighty-four patients with CAA were selected as the research object, used a random number table method, all the patients were divided into the observation group（42 cases） and the control group（42 cases）, the control group was treated with stanozolol tablets, cyclosporin soft capsule medicine treatment, the observation group based on this treatment and was treated with Bushen Shengxue.The two groups were treated for 6 months, compared with the treatment efficiency, the application of enzyme linked immuno double antibody sandwich method for monitoring the serum interferon gamma （IFN gamma）, tumor necrosis factor alpha （TNF alpha） expression levels of the two groups before and after treatment, and measured T cell subsets of the two groups by flow cytometry, and observed the adverse reaction.Results The effective rate of observation group was 92.86%, which was significantly higher than that of the control group （76.19%）, P〈0.05;The levels of IFN-γ, TNF-α and CD8＋ in the observation group were lower than those in the control group （P〈0.05）, the levels of CD3＋, CD4＋ and CD4＋/CD8＋ in the observation group were higher than those in the control group （P〈0.05）;There was no significant difference in the incidence of adverse reactions between the two groups （11.90% vs 9.52%） （P〉0.05）.Conclusions The effect of Bushen Shengxue recipe combined with immune suppression can effectively improve T cell subsets and hematopoiesis regulation factors of the CAA patients, the adverse reactions is light, so it is worthy of clinical application.
