摘  要:目的:探讨加减枳实薤白桂枝汤对大鼠心肌缺血再灌注过氧化损伤的保护作用。方法:将60只SD大鼠随机分为假手术组、模型组、枳实薤白桂枝汤组(4.59g/kg)和加减枳实薤白桂枝汤组(6.48g/kg),使用对应剂量的药物预处理大鼠,假手术对照组和模型对照组给予蒸馏水,14天后采用冠状动脉结扎缺血30min再灌注120min的方法,建立心肌缺血再灌注模型,假手术组开胸不结扎。伊文斯蓝-TTC双染色法测定心肌梗死面积,HE染色观察组织形态学变化,Elisa检测血清中NO、MDA、SOD的含量及心肌组织中MDA、SOD的含量,Western blot检测心肌组织中Akt、p-Akt、eNOS、p-eNOS、cGK1蛋白的含量;免疫组化检测心肌组织peNOS蛋白的表达。结果:与假手术组相比,模型组的心脏梗死面积增大,心肌组织形态紊乱,血清及心肌组织中MDA的含量升高,SOD、NO的含量降低,心肌组织中p-Akt、p-eNOS、cGK1蛋白的含量降低;与模型组相比,枳实薤白桂枝汤(4.59g/kg)和加减枳实薤白桂枝汤(6.48g/kg)预处理组的心脏梗死面积减小,组织形态学改变较小,血清及心肌组织中MDA的含量降低,SOD、NO的含量升高,心肌组织中p-Akt、p-eNOS、cGK1蛋白的含量升高。结论:加减枳实薤白桂枝汤能减轻缺血再灌注出现的心肌梗死,减轻心肌过氧化损伤,其机制可能与PI3K-Akt-eNOS上调NO/cGMP/PKG信号通路有关。Aim : To investigate the protective effect of Modified Zhishixiebaiguizhi Docoction on peroxidation injury in myocardial ischemia/reperfusion rats. Methods: 60 healthy male SD rats were randomly divided into the control group, the model group, Zhishixiebaiguizhi Docoction group(4.59g/kg) and Modified Zhishixiebaiguizhi Docoction group(6.48g/kg). The corresponding dose of the drug pretreated with rats for14 days, while rats in the control group and model control group were given distilled water. Rats in the sham operated group received thoracotomy without ligation, while rats in other groups received coronary artery ligation, and ischemia for 30 mins, reperfusion for 120 mins. Evan's blue - TTC double staining method was used to detect myocardial infarction areas. HE staining was used to observe morphological changes. Contents of NO, MDA, SOD in serum and MDA, SOD in myocardial tissue were detected by Elisa. Contents of Akt, p-Akt, eNOS, p-eNOS, cGK1 protein in myocardial tissue were determined by western blot method and the expression of p-eNOS was measured by immunohistoehemical staining. Results: Compared with the sham operated group, cardiac infarct size in the model group increased, myocardial morphology disordered, MDA in serum and the tissue increased, NO, SOD in serum and the tissue decreased, p-Akt, p-eNOS and cGK1 in the myocardium tissue decreased. Compared with the model group, cardiac infarct sizes decreased in Zhishixiebaiguizhi Docoction group (4.59g/kg) and Modified Zhishixiebaiguizhi Docoction group(6.48g/kg), morphologic changes in myocardial tissue were smaller, MDA in serum and the tissue decreased, NO, SOD in serum and the tissue increased and p-Akt, p-eNOS and cGK1 in myocardium tissue increased. Conclusion : Modified Zhishixiebaiguizhi Docoction has inhibitory effect on myocardial infarction, decrease the peroxidation injury in myocar- dium. The mechanism may related to PI3K-Akt-eNOS up-regulting NO/cGMP/PKG signal pathway.
