正清风痛宁联合玻璃酸钠关节腔注射治疗膝关节炎的近期疗效分析

摘　　要：目的:观察研究正清风痛宁联合玻璃酸钠关节腔注射治疗膝关节炎的近期疗效。方法:选取2014年1月至2015年2月我科室收诊治疗的诊断为膝关节炎的患者90例,按随机数字表法分为治疗组及对照组各45例,对照组患者给予玻璃酸钠关节腔注射治疗,治疗组在对照组治疗基础上加用正清风痛宁治疗,记录并分析治疗前后相关指标变化以及治疗期间不良反应。结果:治疗5周和10周后治疗组患者疼痛视觉模拟(VAS)评分分别为(4.67±1.03、2.25±1.21)分,均明显低于对照组(6.80±1.24、4.13±1.62)分;治疗组治疗5周和10周后的骨性关节炎指数评分(WOMAC)为(45.26±4.19、26.74±2.19)分,均明显低于对照组(51.87±3.49、39.43±3.05)分;治疗组患者治疗后红细胞沉降率(ESR)及肿瘤坏死因子-α(TNF-α)指标为(10.57±1.91mm/h、15.37±5.28pg/ml),均明显低于对照组(14.73±2.68mm/h、31.24±4.69pg/ml);(4)治疗组患者治疗后白细胞介素-6(IL-6)和C反应蛋白(CRP)分别为(49.7±11.7ng/L、3.01±1.82mg/L),均明显低于对照组(64.8±12.8ng/L、5.76±1.7mg/L)。(5)治疗期间,治疗组有13.33%的患者发生不良反应,高于对照组8.89%,但差异无统计学意义。结论:在短期观察时间内,正清风痛宁联合常规西药治疗膝关节炎疗效更好,值得临床进一步研究和应用。Objective： To observe short-term effects of Zhengqing Fengtongning combined with intra-articular injection of sodium hyaluronate on knee arthritis. Methods： 90 patients with knee arthritis from January 2014 to February 2015 were randomly divided into the treatment group and the control group, 45 cases in each grouo. Patients in the control group were treated with intra-articular injection of sodium hyaluronate, while those in the treatment group were treated Zhengqing Fengtongning combined with intra-articular injection of sodium hyaluronate. Relevant indicators were analyzed before and after treatment and adverse reactions during treatment. Results： （1）After 5 and 10 weeks＇treatment, VAS score in the treatment group were （4.67 ± 1.03,2.25 ± 1.21 ） points, which were lower than those in the control group （6.80 ± 1.24, 4.13 ± 1.62） （P 〈0.05） respectively. （2）After 5 and 10 week＇treatment, WOMAC score in the treatment group were （45.26 ±4.19,26.74 ± 2.19） points, lower than those in the control group （51.87 ±3.49,39.43±3.05）respectively （P 〈0.05）. （3）After treatment, levels of ESR and TNF in the treatment group were （14.73 ± 2.68∽∽/h,31.24± 4.69pg/ml） , better than those in the control group （14.73± 2. 68mm/h, 31.24 ± 4.69pg/ml） （ all P 〈 0.05 ） respectively. （4）Levels of IL-6 and CRP in the treatment group were （49.7 ± 11. 7 ng/L,3.01±1.82mg/L） ,lower than those in the control group（64.8 ± 12.8ng/L,5.76 ± 1.7mg/L） （ all P 〈 0.05）. （5）Adverse reactions in the treatment group was 13.33% , higher than that in the control group （8.89%） ,but the difference between two groups was not statistically significant （ P 〉 0.05 ）. Conclusion： The effect of Zhengqing Fengtongning combined with western medicine is better in the short term and it is worthy of further clinical research and application.
