养心生脉颗粒辅助治疗扩张性心肌病并心力衰竭的临床效果观察

摘　　要：目的:观察养心生脉颗粒辅助治疗扩张性心肌病并心力衰竭的临床效果。方法:在2014年3月至2016年2月期间于我院心内科就诊的扩张性心肌病合并心力衰竭患者共72例,随机数字表法分成2组(各36例)。一组为对照组,给予常规治疗药物,另一组为观察组,在常规药物基础上加用养心生脉颗粒用以辅助治疗,分析该两组患者心功能指标和治疗疗效差异。结果:(1)观察组治疗后的总有效率为91.67%,明显高于对照组72.22%。治疗后,观察组血清脑钠肽(NT-Pro BNP)水平、左心射血分数(LVEF)和明尼苏达心力衰竭生活质量量表(ml HFFQ)得分分别为(513.0±14.8pg/ml、44.81±2.03%、49.51±5.24分),均明显优于对照组(669.1±15.2pg/ml、39.95±2.05%、59.82±5.81分)。(3)治疗后,观察组心输出量(CO)、6分钟步行试验(6MWT)和心率分别为(3.16±0.15L/min、474.5±18.5m、85.1±2.2次/min),均明显优于对照组(2.69±0.14L/min、419.5±17.3m、91.6±2.4次/min)。(4)治疗后,观察组左室收缩末期内径(LVSD)和左心室舒张末期内径(LVDD)分别为(41.02±1.86、51.28±2.41)mm,均明显小于对照组(46.80±2.01、58.00±2.59)mm。(5)治疗期间,观察组有11.11%的患者发生不良反应,低于对照组22.22%,但差异无统计学意义。结论:养心生脉颗粒可作为扩张性心肌病并心力衰竭的辅助用药,其可提高治疗疗效,改善患者心功能。Objective： To analyze adjuvant treatment effcets of Yangxin Shengmai Particles to dilated cardiomyopathy with heart failure. Methods ： 72 Dilated cardiomyopathy patients with heart failure from department of cardiology during March 2014 to February 2016 were grouped two groups by random number table method. The control group was given conventional treatment, observation group was given Yangxin Shengmai Particles with conventional treatment. The differences of cardiac function and effect between the two groups were analyzed. Results： After treatment, total effective rate of observation group was 91.67%, higher than control group with 72.22% （P 〈 0.05 ）. After treatment, NT- Pro BNP level, LVEF level and mlHFFQ score of observation group were （513.0±14.8 pg/ml,44.81±2.03 % ,49.51 ±5.24 points）, better than control group with （ 669.1 ± 15.2pg/ml,39.95 ±2.05 % ,59.82 ± 5.81 points） （ all P 〈 0.05）. （3）After treatment, CO,6MWT and heart rate level of observation group were （3.16 ± 0.15 L/min ,474.5 ± 18.5 m,85.1± 2.2 times/min）, better than control group with （2.69 ±0. 14L/min,419.5 ± 17.3m,91.6 ± 2.4 times/min） （all P 〈 0.05 ）. （4）After treatment, LVSD and LVDD of observation group were （41.02 ± 1.86,51.28 ±2.41 ）mm, less than control group with （46.80±2.01,58.00 ±2.59）ram（ all P 〈0. 05）. （5）During treatment,adverse reactions rate of observation group was 11.11% ,lower than that of control group ：22.22%, but the difference was not statistically significant. Conclusion：Yangxin Shengmai Particles can be used as dilated cardiomyopathy and heart failure adjuvant therapy,and it can improve the therapeutic effect and improve cardiac function in patients.
