男性不育症患者早泄的流行病学调查及早泄中医证型分布特点研究

摘　　要：目的:调查男性不育症患者早泄(premature ejaculation,PE)的发病情况及影响因素,通过中医证型分析早泄患者中医证型分布特点研究。方法:对2016年1月至3月在西北妇女儿童医院生殖中心男科门诊诊断为男性不育症的患者进行人口学资料、根据ISSM早泄诊断标准及早泄诊断量表(PEDT)的流行病学调查,确定相关影响因素与其的关系且将结果进行统计学分析。通过《中医男科学》教材中关于早泄中医证型分型论治设计调查表,分析早泄患者中医证型分布特点进行研究。结果:纳入的875例男性不育症患者中有735人完成了PEDT量表,统计量表共有112例患者PEDT分值大于11分,结合阴道内射精潜伏期(IELT)及控制力情况被判断为早泄患者;112例早泄患者均通过《中医男科学》教材中关于早泄中医证型分型论治设计调查表调查,中医证型分布特点如下:肾气不足7例(6.25%),肝经湿热15例(13.4%),心脾两虚33例(29.5%),阴虚火旺42例(37.5%),心肾不交15例(13.4%)。结论:男性不育症患者射精情况,特别是早泄情况值得关注;通过中医证型分析早泄患者中医证型分布特点,心脾两虚型和阴虚火旺型早泄患者较多,为临床的中西医结合治疗打下理论基础。由于此项研究样本量较少在分型上可能有偏移,之后需要扩大样本量及多中心的研究。Objectives：To investigate the incidence and influencing factors of premature ejaculation（PE） in male infertility patients and the characteristics of TCM syndrome types in patients with PE.Methods：Male infertility patients diagnosed in the Northwest Women and Children’s Hospital Reproductive Center from January to March 2016 were surveyed demographically and epidemiologically according to the ISSM diagnosis standards and PEDT,to determine the influencing factors and their correlation.The PE TCM syndrome type design questionnaire in the Male Science of Traditional Chinese Medicine was adopted to analyze the characteristics of TCM syndrome types in patients with PE.Results：Among the 875 male infertility patients,735 ones completed the PEDT scale,and the PEDT score of 112 patients was greater than 11 points,who were diagnosed as PE patients.All the 112 patients underwent TCM syndrome types survey and found 7 cases（6.25%） of Deficiency of Kidney-QI Type,15 cases（13.4%） of Dampness-Heat of Liver Channel Type,33 cases（29.5%） of Heart-Spleen Deficiency Type,42 cases（37.5%） of Yin Deficiency Fire Excess Type and 15 cases（13.4%） of Heart-kidney Imbalance Type.Conclusion：Male infertility patients should pay attention to PE which are mainly Heart-Spleen Deficiency Type and Yin Deficiency Fire Excess Type according to the TCM syndrome types.But due to the small sample size of this study which is less likely to be offset the classification,it is necessary to expand the sample size in the future study and multi-center study is recommended.
