马来酸左旋氨氯地平联合氢氯噻嗪对老年高血压患者靶器官亚临床病变的影响

摘　　要：目的探讨马来酸左旋氨氯地平联合氢氯噻嗪对老年高血压患者靶器官亚临床病变的影响。方法选取原发性老年高血压患者78例，按随机数字表法分为两组，每组39例。对照组给予马来酸左旋氨氯地平治疗，观察组实施马来酸左旋氨氯地平联合氢氯噻嗪治疗。比较两组血管病变检测结果、血压动态检测及心肾功能变化情况。结果治疗后，观察组心踝血管指数、颈动脉内中膜厚度、24h平均血压及24h血压变异率中收缩压、舒张压水平均明显低于对照组，肱动脉血管扩张、夜间血压下降率中收缩压（SBP）、舒张压（DBP）明显升高，差异有统计学意义（P〈0．05）；观察组左心室质量指数、24h尿微量白蛋白、尿白蛋白／血肌酐、左心室射血分数、内生肌酐清除率明显优于对照组，差异有统计学意义（P〈0．05）。结论给予老年高血压患者马来酸氨氯地平联合氢氯噻嗪治疗有助于患者稳定降压，促进靶器官亚临床病变改善。Objective To investigate the effect of levoamlodipine maleate combined with hydrochlorothiazide on subclinical pathology of target organ in elderly with hypertension. Methods Seventynine elderly with primary hypertension treated in hospital were divided into two groups by random number table, with 39 cases in each group. The patients in control group were given levoamlodipine maleate, ob- servation group took levoamlodipine maleate combined with hydrochlorothiazide. The detection results of vascular disease, ambulatory blood pressure monitoring, changes of heart-kidney function of two groups were compared. Results After treatment, the CAVI, IMT, 24 h mean blood pressure, SBP and DBP levels in 24 h blood pressure variety ratio in observation group significantly decreased compared with con- trol group, and the FMD, SBP and DBP in Night blood pressure fall rate significantly increased （P 〈 0.05 ）. The LVMI, 24 h MUA, ALB/SCr, LVEF, Ccr of observation group were significantly better than those of control group （P 〈 0. 05）. Conclusions Levoamlodipine maleate combined with hydrochlorothiazide for elderly with hypertension can facilitate stable decompression and promote improvement of subclinical pathology of target organ.
